翼状胬肉术后巩膜并发症的临床分析及治疗观察

摘　　要：目的分析不同术式翼状胬肉切除术后巩膜并发症的临床表现、原因及治疗。方法回顾性研究翼状胬肉手术治疗且未再次复发的患者200例(200眼),原发性翼状胬肉153眼,复发性者47眼。根据术式的不同分为3组,分析各组的临床表现,可能原因及治疗效果。结果术后出现巩膜局部轻度软化者24眼,14眼经停用皮质类固醇类及抗代谢类滴眼液后恢复;10眼经停用滴眼液及加用营养类药物后治愈。出现巩膜融解坏死者3眼,行羊膜覆盖治愈。结论因术式或术中术后用药等可导致胬肉切除术后出现巩膜并发症,早期诊断和有效治疗对改善其后果至关重要。Objective To observe the clinical manifestations,causes and treatments of different surgical complications of sclera after pterygium excision surgery. Methods A retrospective analysis of 200 cases（200 eyes）of pterygium without relapse after surgery was done.In which 153 eyes were primary pterygium and 47 eyes were recurrance ones.They were divided into three groups according to the surgical methods.The clinical manifestation,causes and effect were observed. Results There were 24 eyes of mild scleral softening.14 eyes recovered by stop using corticosteroid and anti-metebolic eyedrops. 10 eyes were cured by plusing nutritial medicine.Scleral necrosis occurred in 3 eyes,they were cured after amniotic membrane transplantation. Conclusion Surgical treatment or surgery and postoperative medication and other factors could lead to postoperative sclera complications after pterygium excision.Early diagnosis and effective treatments to improve the prognosis of this kind of surgery is essential.
