摘  要:[目的]观察大承气汤对重症急性胰腺炎(SAP)大鼠小肠黏膜分泌性IgA(SIgA)及CD3、γδT细胞的影响,探讨大承气汤治疗SAP的作用机制。[方法]采用胰胆管逆行注射5%牛磺胆酸钠法建立大鼠SAP模型。将SD大鼠随机分为假手术组、SAP模型(模型)组和大承气汤治疗(治疗)组,观察各组不同时间点小肠黏膜病理改变,SIgA水平和CD3、γδT细胞百分率的变化。[结果]模型组与假手术组比较,小肠黏膜损伤指数增加,SIgA水平降低(P<0.01),CD3、γδT细胞百分率降低(P<0.05,<0.01)。治疗组与模型组比较,小肠黏膜损伤指数明显下降(P<0.01),SIgA水平有所增加(P<0.05),CD3、γδT细胞百分率明显上升(P<0.01)。[结论]大承气汤通过影响肠黏膜SIgA分泌和CD3、γδT细胞表达,对SAP大鼠肠黏膜免疫屏障功能具有一定的保护作用。[Objective]To observe the effects of Dachengqi decoction(DD)on SIgA and CD3 γδT lymphocytes in small intestinal mucosa of severe acute pancreatitis(SAP)rats.[Methods] Forty-five SD rats were randomly divided into sham operation group,SAP group and DD group. SAP was induced by retrograde injection of 5%sodium taurocholate.The small intestinal mucosa pathologic change,SIgA level and percentage of CD3,γδT lymphocytes were detected at different intervals.[Results]Compared with the sham operation group,the model group showed increased damage index of small mucosa significantly(P0.01),decreased SIgA level and percentage of CD3,γδT lymphocytes in small mucosa(P0.05or0.01).After the treatment with DD,the damage index of small mucosa was decreased significantly(P0.01),SIgA level increased(P 0.05),the percentage of CD3andγδT lymphocytes were increased significantly(P0.01).[Conclusion]DD had protective effect on small intestinal mucosa immunologic barrier of SAP by regulating the level of SIgA and the expression of CD3,γδT lymphocytes.
