肝动脉解剖变异在肝癌介入治疗中的临床意义

摘　　要：目的用血管造影技术研究肝动脉解剖变异情况,并讨论其在肝癌介入治疗中的影响。方法收集自2001年1月-2009年12月做肝动脉化疗栓塞术(TACE)和肝动脉栓塞术(TAE)的中晚期肝癌患者385例,采用seldinger经皮股动脉穿刺插管的方法,对385例肝癌患者行肝动脉造影,根据肝动脉起源和动脉走行分布情况等解剖特征,分析血管造影图像。结果肝动脉典型分布者275例(71.4%),肝动脉变异者110例(28.6%)。病灶单纯由肝动脉供血238例,147例存在除肝动脉外的多支血管供血。结论肝动脉变异种类繁多复杂,变异发生率较高,应仔细造影观察有无肝动脉变异,尽可能做到插管高选择性,了解肝动脉解剖变异有助于提高插管的成功率和肝癌的介入治疗疗效。Objective To investigate the anatomic variants of hepatic arteries and its elinieal influence on interventional therapy of hepatic carcinoma. Methods This article collects middle-late liver 385 eases retreated by hepatic arterial ehemoembolization or hepatie artery embolization during January 2001 to Deeember 2009 in our hospital X-ray department. 385 eases with hepatic carcinoma.were performed by hepatic angiography after the technology of Seldinger. The arterial phase images were analyzed according to the origin of hepatic artery and other anatomie characteristics of the distribution lines. Results The distribution of hepatie artery was showed 275 eases（71.4% ） typical and 110 eases（28.6% ） variation respeetively. 238 eases had simple hepatie arteries, 147 eases had multiple feeding vessel. Conclusion It was obviously that the variations of hepatic artery were complex and common, and influenced the effect of interventional therapy of hepatic carcinoma. Arterioradiography should he performed carefully, once variant arteries were found, optimal selective eatheteriation should be performed as soonly as possible. For the elinie familiar with the blood supply of liver cancer,recognizing anatomie variation of hepatic artery eatheterizatian improved the cure rate and effect of interventional treatment of liver cancer.
