摘　　要：目的观察喉全切除术后应用Blom-Singer发音管重建喉发声功能的效果。方法回顾性分析2002年2月至2009年4月15例喉全切除术后行Blom-Singer发音管重建喉发声功能患者的临床资料。结果 15例患者中10例行Ⅰ期发音管重建术,全部成功;5例行Ⅱ期发音管重建术,亦均成功。结论喉全切除术后应用Blom-Singer发音管重建喉发声功能是一种有效的发声重建方法,适应症广;保留气管食管壁的完整是手术成功的关键;下咽黏膜保留越多新声门振动发声效果越好;放疗、咽瘘等因素不影响发声效果;清洗保养是延长Blom-Singer发音管寿命的关键。Objective To observe the results of the voice rehabiliation after total laryngectomy.Methods An retrospective analysis of the clinical data of the 15 patients implanted with Blom-Singer voice prostheses after total laryngectomy from February 2002 to April 2009.Results Among the 15 patients,10 cases with voice rehabilitations in Stage Ⅰ were successful and five cases with voice rehabilitations in Stage Ⅱ were successful too.Conclusion The implantation of Blom-Singer device after total laryngectomy is simple and effective and broad indications.The key to a successful surgery is to retain the completeness of the tracheo-esophageal wall.the more hypopharyngeal mucosa is retained,the better the new glottis will vibrate,and the better the voice will be.Radiotherapy and pharyngeal fistula do not affect voice production.Proper cleaning and maintenance will extend the service of the voice protheses.
