脑出血急性期MMP-9、IL-6、TNF-α的临床动态观察与相关性研究

摘　　要：目的通过测定高血压脑出血患者颅内血肿液及静脉血清中基质金属蛋白酶-9(MMP-9)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,了解这些炎症因子与脑出血病灶及外周血之间的关系。方法在高血压脑出后24h,72h,7d和14d取80例患者静脉血清及血肿液检测MMP-9、IL-6、TNF-α的含量(实验组),并与30例正常体检的患者(对照组)对比。结果高血压脑出血患者血肿液MMP-9、IL-6、TNF-α含量显著高于对照组人群静脉血,而且还显著高于实验组患者本身静脉血,实验组患者静脉血清中MMP-9、IL-6、TNF-α含量显著高于对照组人群静脉血清且病情越重几种炎症因子水平越高。脑出血不同时期静脉血清MMP-9、IL-6、TNF-α含量比较,在72h最高,此后呈下降趋势。结论高血压脑出血患者MMP-9、IL-6、TNF-α参与了脑出血的炎症反应,是脑出血后脑组织损伤的重要机制之一。Objective To detect the levels of matrix metalloproteinase-9（MMP-9）,interleukin-6（IL-6）,tumor necrosis factor-α（TNF-α） in hematoma fluid and venous serum in patients with hypertensive cerebral hemorrhage and to study the relationship of cytokine and venous serum and hematoma fluid in patients with hypertensive cerebral hemorrhage.MethodsLevels of MMP-9,IL-6,TNF-a in venous serum and hematoma fluid in 80 cases（test group） 24h,72h,7d and 14d after encephalorrhagia were measured,and compared with those of 30 cases with normal physical examination（control group）.ResultsThe levels ofMMP-9,IL-6,TNF-a in hematoma in patients with hypertensive cerebral hemorrhage were higher than those in venous serum in test group and normal control group.The levels of MMP-9,IL-6,TNF-tt in venous serum were significantly higher than those in normal control group,and increased with an increase of severity of patient＇s condition.When compared the levels of MMP-9,IL-6,TNF-α at different stages of hypertensive cerebral hemorrhage,the highest levels were detected at 72h followed by a tendency to decrease after 72h.Conclusion MMP-9,IL-6,TNF-α have participated in the inflammation reaction in hypertensive cerebral hemorrhage,which could be one of significant mechanism in brain tissues injury after encephalorrhagia.
