间变大细胞淋巴瘤合并噬血1例分析及文献复习

摘　　要：目的提高以不明原因发热及合并噬血为主要表现的间变大细胞性淋巴瘤的诊断水平,并探讨其临床特点及治疗方法。方法回顾性分析1例经病理确诊为间变大细胞性病例的临床资料,并复习相关文献来提高对该病的认识。结果该患者以不明原因发热就诊,最后通过骨髓活检病理检查确诊,先后采用CHOP、VDLP方案化疗,病情暂时控制,继续随访治疗中。结论以不明原因发热为主要表现的间变大细胞性淋巴瘤临床表现缺乏特异性,易延误诊断,确诊取决于淋巴结、骨髓、脾等组织病理学证据。Objective To increase the diagnostic level in anaplastic large cell lymphoma with unknown origin fever and hemophagocyte,approach the clinical features and treatment.Methods Analysis retrospective the pathologically diagnosed of one case anaplastic large cell lymphoma,review the pertinent literature for increasing recognition of the disease.Results The patient was diagnosed for bone marrow biopsy and unknown origin fever,treated with CHOP、VDLP chemotherapy,the disease was controled temporary.Conclusion Anaplastic large cell lymphoma with the main performance of unknown origin fever is lack of specificity,and easy to misdiagnosis,the diagnosis of this disease depends on the evidence of histopathology in lymph nodes、bone marrow and spleen.
