摘　　要：目的研究丘脑肿瘤的临床特点和手术治疗效果。方法回顾性分析经开颅手术治疗的37例丘脑肿瘤病人的临床资料。对这些病人中29例进行了2个月~6年的随访,8例失访。结果全切20例,次全切14例,大部切除3例。术后神经缺失症状和体征改善者27例,无明显变化者5例,加重者4例,死亡1例。随访的29例病人中,可正常生活工作者16例,生活能自理者5例,严重残障者2例,死亡6例。结论同多数脑肿瘤患者相比丘脑肿瘤低龄者中较多,肿瘤性质多为低级别星形细胞瘤,积极的开颅手术和恰当的辅助治疗对这部分病人的预后是有利的。结合患者临床表现和应用现代神经影像学技术。Objective To study the clinical characteristics of the thalamic tumors and the curative effects of surgery on them.Methods The clinical data of 37 patients with thalamic tumors,who underwent craniotomy from January,2001 to January,2007,wereanalyzed retrospectively.Twenty-nine patients followed up from 2 months to 6 years after the surgery.Results There were 26 males and11 females,and their ages ranged from 2 to 61 years in 37 patients with thalamic tumors.The main clinical manifestations were causedby intracranial hypertension.Of 37 patients with thalamic tumors,20 received total resection of the tumors,14 subtotal and 3 greatlypartial.The clinical symptoms were improved in 27 patients,unchanged in 5,aggravated in 4 and 1 died.Of 29 patients followed up,16 returned to bormal living and work,5 could take care of themselves in living,2 were severely disabled and 6 died.ConclusionsThalamic tumor was more common in the children than that in the elder adults.The low-grade astrocytomas predominated in thethalamic tumors.The microsurgery combined with suitable auxiliary treatment including chemotherapy,radiotherapy and so on is helpfulto improvement of the prognosis in the patients with thalamic tumors.The postoperative complications may be decreased,the quality oflife may be improved and the survival time may be prolonged by the application of neuronavigation to the microsurgery for the thalamictumors under the guidance of contemporary neuroimaging combined with the clinical manifestation in the patients with thalamic tumors.
