适形放疗结合体部γ刀照射在非小细胞肺癌放疗中的应用

摘　　要：目的:探索适形放疗结合体部γ刀照射在非小细胞肺癌放射治疗中的应用。方法:对21例中心型非小细胞肺癌患者行适形放疗结合体部γ刀照射,适形照射40-54Gy/20-30f/4-6W,1.8-2.0Gy/f,同期或贯续行体部γ刀照射,50%剂量曲线10-15Gy/4-10f,同期1.0-1.5Gy/f,贯续2.5-3.0Gy/f。结果:所有患者均完成放疗计划,症状缓解17例,总有效率80.95%(17/21)。全组发生放射性肺炎Ⅰ级4例,Ⅱ级2例,无Ⅲ、Ⅳ级放射性肺炎发生。结论:适形放疗结合体部γ刀照射治疗中心型非小细胞肺癌近期疗效好,不良反应轻,是一种有效的局部治疗方法。Objective：To study the application of combination of conformal therapy with body γ-knife radiotherapy in patients with non-small cell lung cancer（NSCLC）.Methods：Combination of conformal therapy with body γ-knife radiotherapy was performed on 20 patientis with NSCLC.The dosage of conformal therapy was 40-54Gy/2030f/4-6W,1.8-2.0Gy/f,meanwhile or follow body γ-knife radiotherapy was 10-15 Gy /4-10f with 50% isodose curve.Results：All patients were completed radiotherapy.The symptom complete response was observed in 17 cases,the overall response rate was 80.95%（17/21）.Patients developed radiation pneumonitis Grade I in 4 cases,Grade II 2 cases,no Grade III,Ⅳ pneumonitis occurred.Conclusion：Combination of conformal therapy with body γ-knife radiotherapy was an effective local treatment option for NSCLC patients with high response rate and minimal toxicity.
