放疗同步化疗治疗直肠癌术后局部复发或盆腔转移的疗效观察

摘　　要：目的:观察三维适形放疗(3DCRT)结合奥沙利铂为主化疗治疗直肠癌术后局部复发或盆腔转移的临床疗效。方法:回顾分析近3年来收治的直肠癌术后局部复发或盆腔转移患者32例,应用3DCRT2Gy/次,Dt:56-66Gy,同步奥沙利铂、氟尿嘧啶、亚叶酸钙化疗,21天为1个周期,放疗结束后再化疗2-3个周期。结果:完全缓解率25%(8例),部分缓解率59%(19例),有效率84%,疼痛缓解率91%(29例)。1、2年生存率分别为84%、63%,生活质量明显改善,无治疗相关性死亡。结论:3DCRT结合奥沙利铂为主化疗治疗直肠癌术后局部复发或盆腔转移,可提高局部控制率和延长生存期,不良反应可耐受。Objective：To study the outcome of local advanced or recurrent rectal cancer treated by three dimensional conformal radiation therapy（3DCRT） combined with oxialiplation＋5-flurouracil＋calcium folinat chemotherapy.Methods：Between June 2004 and August 2007,32 patients with loco-regionally recurrent or mastastic rectal cancer were treated by 3DCRT of 56-66Gy in 2Gy fractions,chemotherapy was given concurrently using oxaliplation,5-flurouracil and calcium folinat with iv drip on D1-D5 in two cycle（21days as one cycle）,3-4 weeks after radiochemotherapy,chemotherapy was continued for 2-3 cycles.Results： After the follow-up of 12-35 months,the total response rate,complete response rate and partial response rate were 84%（27/32）,25%（8/32） and 59%（19/32）,respectively.The painrolief rate and the mean painreliefon time were 91% and six months.The 1-2 year survival rates were 84%and 63%.Life quality was improved without any treatment related death.Conclusion： 3DCRT combined with concurrent chemotherapy is effective and well-tolerated in patients with post-operatively locoregionally recurrent or pelvic mastastatic rectal cancer.
