微波热疗中细胞杀死量与前列腺体积变化的关系

摘　　要：目的研究经Prostalund尿道反馈微波热疗(PLFT)中,计算的细胞杀死量与前列腺体积变化之间的相关性。方法选取行PLFT治疗的良性前列腺增生患者40例,比较增生细胞杀死量和前列腺体积的变化。细胞杀死量通过PLFT系统软件自动计算。前列腺体积按照经直肠超声(TRUS)求积法来计算,分为治疗前及治疗后3个月、6个月、12个月的随访值。用配对t检验来分析细胞杀死量和前列腺体积变化差异的统计学意义。线性回归来推断细胞杀死量和经直肠超声测量前列腺体积变化之间的关系。结果治疗前(n=40)平均前列腺体积是57.4 cm3,治疗后3个月(n=40)、6个月(n=36)、12个月(n=32)前列腺平均体积分别是45.1、45.1、46.9 cm3,相应的细胞杀死量分别是14.6、15.11、5.2 cm3。计算的细胞杀死量大于术后3、6、12个月随访的前列腺体积缩小量,且具统计学意义(P<0.005),并且在3、6和12个月显示出强的相关性(r=0.936,P<0.005)。结论 PLFT方法中细胞杀死量与TRUS测量的前列腺体积缩小成比例,系数是1.1;细胞杀死量可以预测治疗后前列腺体积的变化。Objective To study the correlation between the prostate volume change and cell-kill volume in Prostalund transurethral feedback microwave thermotherapy（PLFT）.Methods 40 patients with benign prostatic hyperplasia（BPH） treated with PLFT method were enrolled in our study,The calculated cell-kill from the patients was compared to the post-treatment prostate volume change.The cell-kill volume was calculated by the device of the PLFT software.The prostate volume was estimated with transrectal ultrasound（TRUS） planimetry at baseline,3,6,and 12 months follow-up.A paired t-test was used to test the statistical significance of differences between the cell-kill volume and the prostate volume change.Linear regression was used to infer a relationship between the cell-kill volume and the prostate shrinkage.Results The mean prostate volume at baseline（n=40） was 57.4cm3.After 3（n=40）,6（n=36） and 12（n=32） months,it was 45.1,45.1 and 46.9cm3,respectively.The corresponding average cell-kill volume was 14.6,15.1 and 15.2cm3,respectively.Calculated cell-kill volume was significantly larger than prostate shrinkage at 3,6,and 12 months（P0.005）,and showed a strong correlation at 3,6 and 12 months（r=0.936,P0.005）.Conclusions Cell-kill volum of the PLFT method are proportional to the TRUS prostate shrinkage by a factor of 1.1 and the cell-kill can predict the shrinkage of the prostate after the treatment.
