摘　　要：目的观察瘤周皮下注射亚微米级脂质微泡(SLMs)对肿瘤周边引流淋巴结的超声造影显影效果。方法建立兔VX2乳腺癌模型10只,瘤周皮下注射SLMs1ml并局部按摩后,按照二次谐波造影显像、彩色多普勒超声爆破显像和再次二次谐波超声造影的顺序,观察造影剂聚集的可疑淋巴结,并进行体表标记,取该结节活检并行病理检查。结果皮下注射SLMs并局部按摩后,二次谐波造影见腋窝处的造影剂结节样聚集增强。彩色多普勒模式下,该区呈瞬间爆炸样花色血流信号。再次在造影模式下观察该区域,见结节样增强明显减弱甚至消失。再次按摩注射部位约2～5min后,该区域第二次被造影剂充填。病理检查提示造影剂聚集区域为正常淋巴结组织或有微小转移灶的淋巴结,淋巴结内大片增强缺损区提示有肿瘤大范围侵犯和转移。结论皮下注射SLMs后,能清晰显示造影增强肿瘤周边引流淋巴结,通过造影增强-彩色爆破-再增强方法可有效检出肿瘤引流淋巴结。Objective To investigate development effect of draining lymph nodes of VX2 tumor with contrast-enhanced ultrasonography using injection of submicron lipid microbubbles（SLMs）.Methods VX2 breast cancer models of 10 rabbits were constructed and 1 ml SLMs were administered subcutaneously around tumor in each rabbit.After the injection-sites were massaged,contrast-enhanced ultrasonography（CEUS）,color Doppler flow imaging（CDFI）and a second CEUS were performed.The suspicious lymph nodes with nodular contrast enhancement were labeled and surgically removed for histologic examination.Results After subcutaneous administration of SLMs and regional massage,CEUS could detect contrast enhanced nodular structures in the axillary region.CDFI showed blooming effect and transient mosaic color.The nodular structure indicating lymph nodes could be refilled and enhanced again during second massage in 2—5 min of the same injection-sites.Histological examination found the CEUS regions as lymph nodes with or without minute tumor metastasis,while the lymph nodes with large defective enhancement area usually located under the condition of wide metastasis or necrosis.Conclusion Subcutaneous injection of SLMs combined with CEUS can image and detect the draining lymph nodes of VX2 tumor with the contrast,color blooming and contrast method.
