摘 要:目的研究化疗及导向化疗对大肠癌组织及细胞的影响.方法大肠腺癌患者37例,根据是否注射化疗药物分成A,B两组.A组17例,术前不注射任何化疗药物或化疗药物的偶联物,将其作为对照组;B组20例,其中7例手术前3div丝裂霉素C(MMC8mg),13例手术前iv导向化疗药物抗CEA单克隆抗体-MMC交联物(含MMC8mg,抗CEA单克隆抗体10mg);其后对所有37例患者行手术治疗并切取标本进行病理组织学观察,AgNOR颗粒检测及肿瘤浸润淋巴细胞分类及计数.结果A组病理退变程度17例(1000%)均为1级;B组病理退变程度1级4例(200%),2级12例(600%),3级4例(200%),两组之间差异有高度显著性(P=0000000376).A组癌细胞AgNOR颗粒/核均数为411颗;B组癌细胞AgNOR颗粒/核均数为258颗,其差异有高度显著性(P<001,t=5143).肿瘤浸润淋巴细胞以T细胞CD+3为主,同组中T淋巴细胞CD+3(aP<001,t=20229)与B淋巴细胞CD+20之间的差异有高度显著性(bP<001,t=19761);A组T淋巴细胞CD+3为6091%,B组T?AIM To study the effect of chemotherapy and targeting chemotherapy on colorectal carcinoma (CRC). METHODS Thirty seven patients were divided into two groups: group A (17 patients, without preoperative chemotherapy) and group B (20 patients, receiving preoperative chemotherapy, targeting chemotherapy and mitomycin C 8 mg ). Resected specimens of colorectal carcinoma were assessed histologically by silver staining protein score of the nucleolar organizer region (AgNOR) and by classification and counting of infiltrating lymphocytes to study apoptosis of neoplastic cells caused by chemotherapy and targeting chemotherapy. RESULTS In group A, all resected tumor tissues of 17 patients were in grade 1 retrograde degeneration, but in group B, 4 cases in grade 1, 12 cases in grade 2 and 4 cases in grade 3. There was a significant difference between the two groups ( P =0 000000376). Counts of AgNOR granules per nucleus in group A were 4 11 and 2 58 in group B with a significant difference ( t =5 143, P <0 01). The infiltrating lymphocytes were predominantly T lymphocytes (CD + 3), with a significant difference in a same group between counts of T lymphocytes ( t =20 229, a P <0 01) and B lymphocytes (CD + 20 ) ( t =19 761, b P <0 01). T lymphocytes was 60 91% in group A and 72 34% in group B. B lymphocytes was 10 78% in group A and 12 65% in group B. CONCLUSION Chemotherapy and targeting chemotherapy could cause retrograde degeneration of tumor tissues, decrease counts of AgNOR granules per nucleus of tumor cell, and cause the apoptosis of tumor cells. Infiltrating lymphocytes were predominantly T lymphocytes (CD + 3) after chemotherapy and targeting chemotherapy.
