短疗程大剂量加速超分割放疗晚期恶性肿瘤的疗效观察

摘　　要：目的观察短疗程大剂量加速超分割放疗对晚期恶性肿瘤的近期疗效。方法晚期恶性肿瘤２０例，采用每次剂量３７０ｃＧｙ，每日２次，每次间隔≥６ｈｒ，连续照射２天为一疗程，每３周后可重复１个疗程，肿瘤灶剂量１４８０～４４４０ｃＧｙ／４～１２次／３～９周。结果肿瘤灶有效率为４５．８％，生活质量改善率７０％，主观症状改善率８０％。全组生存期１～２２个月，平均生存期５．２个月，３例存活期超过１年，其中２例分别超过１６个月和２２个月。放疗副反应较轻。结论短疗程大剂量加速超分割分段放疗对晚期恶性肿瘤具有较好的姑息治疗效果。Objective To assess palliative effects on advanced malignancies with the short-course rapid hyper fractionated irradiation.Methods Twenty patients with 24 lesions were entered into the non-randomized study protocol.The protocol consists of twice a day fraction (370 cGy per fraction) and they were separated ≥6hr for 2 consecutive days totalling 1480 cGy per couse.This couse was then repeated three times every 3 week for tumor dose of 4440 cGy.Results Tumor lesions had partial response 45.8% .Seventy percent of patients received the good quality of life.Eighty percent cases had subjective response.The average survival after completion of therapy was 5.2 months (ranging from 1～22 months).Three patients survival exceed one year.The acute reactions were minimal.Conclusion The short-course rapid hyper fractionated irradiation has palliative effects on advanced malignancies.
