摘 要:目的 通过对脑动静脉畸形(AVM)出血与相关血管形态构筑关系的分析,预测AVM发生出血的可能性,从而降低AVM出血的风险和造成的危害.方法 收集104例AVM患者的造影资料,对AVM的血管构筑形态和出血状况(即AVM供血动脉类型和数目,引流静脉的类型、数目和形态,引流静脉出现、结束的时间及AVM血管团的大小、部位)进行分析.结果 AVM出血与其供血动脉的类型、引流静脉的状况(类型、数目、形态)及畸形血管团的大小和部位具有重要关联,其中椎动脉和穿支动脉供血、单支静脉和深部静脉引流、引流静脉迂曲和损伤、畸形血管团〈3cm及位于基底节、后颅窝的AVM最易出血.结论 AVM出血危险因素与其供血动脉(类型)、引流静脉(形态、支数及部位)、畸形血管团的部位及大小有关,熟悉和掌握AVM出血与其相关血管形态构筑关系,对于降低AVM出血发生率、致残率及病死率具有重要意义.Objective The investigate the relationship of anglo-architecture with hemorrhage of cerebral arteriovenous malformations (AVM). Methods One hundred and four AVM patients diagnosed by digital subtraction angiography were enrolled in the study. The morphology and anglo-architecture of AVM were classified and analyzed, including the type, shape and number of feeding artery and draining vein, the end time of vein drainage, and size and location of AVM. Results The hemorrhage of AVM was associated with the type, number and shape of feeding arteries and drainage veins amd the location and size of AVM. The following factors were associated with the risk of AVM bleeding: the feeding artery was vertebral artery and perforating branches; the drainage vein was single, deep venous, tortuous and injured, the size of AVM was less than 3 cm, and the location of AVM was in basal ganglia and posterior fossa. Conclusion The morphology and angio-architecture of AVM can predict the possibility of AVM hemorrhage.
