原发性胆囊癌中VEGF-C表达及与肿瘤恶性特征的相关性研究

摘　　要：目的 分析原发性胆囊癌组织中血管内皮生长因子C（CVEGF-C）在mRNA和蛋白水平的表达及其与胆囊癌恶性特征的关系.方法 采用逆转录聚合酶链反应（RT-PCR）和免疫组化法在mRNA和蛋白水平检测VEGF-C在原发性胆囊癌和胆囊腺瘤样息肉中的表达情况,并分析VEGF-C免疫组化阳性表达与胆囊癌肝浸润、淋巴转移、病理分级和Nevin分期等肿瘤恶性特征的关系.结果 在胆囊癌和胆囊腺瘤样息肉组织中VEGF-C mRNA阳性表达率为88.23%（15/17）和25%（3/12）,两者的差异有统计学意义（P〈0.05）.免疫组化染色VEGF-C蛋白检测阳性表达率为76.47%（13/17）和25%（3/12）,两者相比差异有统计学意义（P〈0.05）.VEGF-C的阳性表达率在胆囊癌肝浸润、淋巴转移和Nevin Ⅳ、Ⅴ期者明显高于无肝浸润、无淋巴转移和Nevin Ⅰ、Ⅱ、Ⅲ期者（P〈0.05）,而各病理分级（高、中、低分化）的阳性表达率的差异无统计学意义（P〉0.05）.结论 原发性胆囊癌组织中VEGF-C在mRNA和蛋白水平均存在高表达,VEGF-C阳性表达与胆囊癌肝浸润、淋巴转移和Nevin分期明显相关,但与病理分级无明显相关.Objective To investigate the mRNA and protein expression of VEGF-C in primary gallbladder carcinoma （PGC） and its correlation with clinicopathological characteristics. Methods The tissue samples were collected from 17 cases of primary gallbladder carcinoma and 12 cases of gallbladder adenomaous ployps （GAP） from August 2000 to August 2009 in the General Surgery Department of the Zhejiang Provincial People＇s Hospital. VEGF-C mRNA and protein expression were detected by RT-PCR and Immunohistochemistry respectively. The correlation of VEGF-C protein expression with clinicopathological char- acteristics of PGC was analyzed. Results The expression rates of VEGF-C mRNA of PGC and GAP cases were 88.2%（15/17） and 25.0%（3/12） respectively（P〈0.05）. The expression rates of VEGF-C protein in PGC and GAP were 76.5% （13/17） and 25.0% （3/12）, respectively （P〈0.05）. The expression rates of VEGF-C protein in PGC with liver infiltration and lymphatic metastasis, and Nevin stage IV and V were significantly higher than those without liver infiltration and lymphatic metastasis, and stage Nevin I, II and Ill （P〈0.05）. However, the expression rates of VEGF-C was not related to tumor differentiation of PGC （P 〉0.05）. Conclusion VEGF-C mRNA and protein were over-expressed in PGC patients, which is correlated with some clinicopathological characteristics of the tumor.
