摘  要:目的探讨抗生素和微生态制剂对老年非肠道病患者肠道菌的影响。方法采集239例老年患者的大便作大便培养。需要使用抗生素者,随机分成仅给抗生素治疗组和同时应用微生态制剂(整肠生)组,分别在使用抗生素前,使用第5、10及15天采集大便标本,测定肠道菌群。结果抗生素使用初期(5d),肠杆菌、乳杆菌和梭杆菌都明显减少;在抗生素使用10d时和15d后,肠杆菌数量显著上升,而肠道有益菌拟杆菌、双歧杆菌、乳杆菌数量明显减少(P<0.05)。使用广谱抗生素的同时使用微生态制剂,双歧杆菌、拟杆菌的数量不同程度增加;肠道厌氧菌数量增加后,条件致病菌肠杆菌数量随之在15d时明显下降(P<0.05)。老年患者中年龄和双歧菌呈正相关(r=0.358,P<0.05)。结论抗生素的使用可以引起肠道菌群的失调,使用时间越长,肠道菌群受损越大。微生态制剂不同程度抑制抗生素导致的肠道菌群的失调。Objective To investigate the effects of antibiotics and micro-ecological agent on the intestinal flora in the elderly with non-intestinal diseases.Methods Intestinal flora were detected by stool cultures in 239 old patients with non-intestinal diseases.Those who needed antibiotics were randomly divided into two groups of A treated with antibiotics only and M treated with additional micro-ecological agent(bacillus licheniformis capsules).The fecal was collected for detecting intestinal flora before antibiotics use and on the 5th,10th and 15th day during treatment.Results The quantities of Escherichia coli,Lactobacillum and Fusobacterium obviously in grorup A droped on the 5th day(P〈0.05).On the 10th and 15th day,quantity of Escherichia coli rised,but quantities of Lactobacillum,bifidobacterum and bacteroides decreased gradually in group A(P〈0.05).Compared with group A,more quantities of bifidobacterum and bacteroides at different degrees and less quantities of Escherichia coli were found on the 15th day in group M(P〈0.05).There was a positive correlation between bifidobacterum′s quantity and age(P〈0.05).Conclusion Antibiotics therapy may lead to alteration of intestinal flora in a time-dependent manner,which could be inhibited to certain degree by concurrent use of micro-ecological agent.
