早期乳腺癌保留乳头乳晕的皮下乳腺癌改良根治术38例分析

摘　　要：目的探讨行保留乳头乳晕复合体(nipple-areola complex,NAC)的皮下乳腺癌改良根治术同期应用硅凝胶假体植入乳房再造的适应症、手术方法和疗效.方法 2007年8月至2010年7月,对38例乳腺癌行保留NAC的皮下乳癌改良根治术,同时应用硅凝胶假体植入行乳房再造.结果全部再造成功;随访2～36个月(平均17个月),再造乳房效果良好,均无癌局部复发或远处转移.结论保留NAC的皮下乳腺癌改良根治术即时乳房再造,手术操作简单;切口隐蔽,再造乳房形态效果好,能有效提高患者生存质量,对早期乳腺癌患者可推广应用.Objective To investigate the indications,surgical methods and esthetic results of modified radical mastectomy with preserved nipple-areola complex（NAC）and immediate breast reconstruction with silicone prosthesis implants for early breast cancer patients.Method From August,2007 to July,2010,38 early breast cancer patients in our hospital received modified radical mastectomy with preserved nipple-areola complex（NAC）and immediate breast reconstruction with silicone prosthesis implants.Result All patients obtained the successful breast reconstruction,and were followed up for 2～36 months（mean 17 months）,the results showed the morphology of reconstructed breast wre excellent without recurrence or metastasis.Conclusion Modified radical mastectomy with preserved nipple-areola complex（NAC）and immediate breast reconstruction is a simple and practical treatment for patients with early breast cancer,and it can get satisfied esthetic results and improve the life quality of patients.
