子宫外子宫内膜间质肉瘤2例临床病理观察

摘　　要：目的探讨子宫外子宫内膜间质肉瘤(ESS)的临床病理特点、组织发生及鉴别诊断。方法回顾2例ESS的临床及病理资料,并辅以免疫组化分析。结果 2例患者均以盆腔包块和腹痛入院,其中1例伴有子宫内膜异位症。组织学显示瘤细胞由浸润性、弥漫性的单一圆形或梭形细胞组成,类似增生期子宫内膜间质细胞,螺旋小动脉可见。其中1例瘤细胞异型、坏死明显,为低分化ESS。免疫组化:瘤细胞CD10、vimentin、ER和PR均(+)。结论子宫外ESS的发生与异位的子宫内膜相关,CD10、vimentin和PR是标记ESS较好的一组抗体。Objective To explore the clinicopathological features and differential diagnosis of primary extrauterine endometrial stromal sarcoma. Methods The clinical and pathological data of two cases were retrospectively analyzed, combined with immunohistoehemistry. Results Two patients both had abdominal pain and a pelvic mass, one of whom had ectopic endometriosis. Histologically, tumor cells consisted of infiltrative and diffuse round or ovary cell with obvious small helix arteries, similar with proliferative endometrial mesenehymal cells. One case with marked atypia and necrosis of tumor cells was diagnosed as low differentiated endometrial stromal sarcoma. Immunohistochemistry showed the tumor cells were positive for CD10, vimentin, ER and PR in two cases. Conclusion The tumorigenesis of extrauterine endometrial stromal sarcoma is related with etopie endometrium. CD10, vimentin and PR are a panel of markers for endometrial stromal sarcoma.
