摘 要:目的:评价超声检查在新生儿胼胝体发育不全中的应用价值。材料和方法:总结2007-01~2009-12在湖南省儿童医院住院的9663例接受常规颅脑超声筛查的新生儿,发现18例胼胝体发育不全,与MRI比较,评价超声对诊断新生儿胼胝体发育不全的准确性。结果:18例新生儿中超声对胼胝体完全缺如、部分缺如的诊断敏感性分别为100%和80%,特异性分别为84.6%和85%,准确性分别为94.4%和83.3%,与MRI的Kappa值分别为:0.92和0.84。二者的一致率为90%。确诊的18例中有12例合并有10种其他的颅内畸形。结论:超声检查与MRI对诊断新生儿胼胝体发育不全具有高度一致性,故前者可作为筛查新生儿胼胝体发育不全的常规检查方法。Purpose To evaluate the value of ultrasonography(US)in diagnosis of neonatal agenesis of the corpus callosum(NACC).Materials and Methods From Jun 2007to Dec 2009,9663infants were screened for NACC by US.Of the 9663infants,18were found having NACC and consequently recruited in this study.All the 18patients underwent brain Magnetic resonance imaging(MRI)after US examinations.The presence or absence of NACC on US and MRI was determined by two readers separately.The NACC was classified into two subtypes:complete and partial lack of corpus callosum.The subtype of NACC was also evaluated for each infant on US and MRI,independently.The MRI was considered to be a"gold standard"to evaluate the sensitivity and specificity of US for diagnosis of NACC.Results The Kappa value of US vs.MRI for identification of complete and partial lack of corpus callosum was 0.92and 0.84,respectively.The sensitivity and specificity of US was 100%and 84.6%for complete ack of the corpus callosum,and 80% and 85%for partial lack of the corpus callosum,respectively.Compared to MRI,the accurate rate of identifying complete and partial lack of corpus callosum was 94.4%and 83.3%,respectively.The rate of concordance was 90%between US and MRI.Conclusions There is a good agreement between US and MRI in diagnosis of NACC.Compared to MRI,US is a widely available and cheaper alternative modality for evaluation of NACC.and the former method can be acted as routine examination technique for neonatal agenesis of the corpus callosum.
