摘 要:目的 调查海口市院前急救患者流行病学情况,探讨院前急救病例特点.方法 从海口市120急救中心系统数据库中导出2008年度全部数据进行统计分析.结果 ①院前急救组患者的调度时间、到达时间、现场时间、急救半径分别为(2.23±1.18)、(16.41±7.13)、(12.97±6.28)min和(7.26±3.75)km,院前非死亡组患者分别为(2.27±1.12)、(16.85±7.39)、(24.58±12.26)min和(7.21±3.98 )km,院前死亡组患者分别为(2.19±1.06)、(15.97±8.26)、(23.65±11.72)min和(7.32±3.64)km,院前非心脏性猝死组患者分别为(2.21±1.08)、(16.15±8.12)、(23.52±11.66)min和(7.48±3.71)km,院前心脏性猝死组患者分别为(2.16±1.09)、(15.63±7.94)、(24.39±12.14)min和(6.96±3.83)km,各组在调度时间、到达时间的差异均无显著性(P〉0.05),但院前非死亡组患者的现场时间明显少于院前死亡组患者(P〈0.001).②各组患者均以第一季度为最多.③在9729例院前急救患者中,创伤类患者是院前急救中最多的,主要为21~50岁青壮年,其次分别是循环系统、神经系统患者,且均以51岁(尤其是70岁)以上的中老年患者居多.④循环系统、神经系统、呼吸系统患者死亡分别位居第一、第二、第四,均以51岁(尤其是70岁)以上的中老年患者居多,其中心脏性猝死又是循环系统的主要死亡原因 创伤类的死亡位居第三,年龄以21~50岁青壮年患者居多.⑤男性院前急救的数量、总死亡量及院前心脏性猝死均明显高于女性.结论 ①加强心脑血管病和呼吸系统疾病防治,提高中老年患者常见急危重症的早期识别与院前急救水平对降低死亡有重要意义.②加强安全生产、遵守交通法规、强化法制意识将是降低创伤,尤其是交通意外伤发生及其死亡的有力手段.Objective To investigate the epidemiological information of patients with pre - hospital medical care in Haikou city and to probe into the patients' characteristics. Methods The data in 2008 were exported from the computer database of Haikou emergency medical center. Results (1)The scheduling time, running time, rescue time and service radius of patients in the pre - hospital medical care group were (2.23 ± 1. 18 ) , ( 16.41 ± 7.13 ) , ( 12.97 ± 6.28 ) min and (7.26 ± 3.75 ) km, and the above parameter of patients in the non - death group were ( 2.27 ± 1. 12 ), ( 16.85 ± 7.39 ), (24. 58 ± i2.26) rain and (7.21 ±3.98) km, and the above parameter of patients in the death group were (2.19 ±1.06), (15.97 ±8.26), (23.65 ±11.72) min and (7.32 ±3.64) km, and the above parameter of patients in the non - sudden cardiac death group were (2.21 ± 1.08), ( 16.15 ± 8.12), (23.52 ± 11.66) rain and (7.48 ± 3.71 ) km, and the above parameter of patients in the sudden cardiac death group were ( 2.16 ± 1.09 ) , ( 15.63 ± 7.94 ) , (24.39 ± 12.14 ) rain and (6.96 ± 3.83 ) km respectively. Although the scheduling time, the running time didnt have significant difference among all groups ( P 〉 0.05 ), the rescue time in the non - death group was less than that in the death group (P 〈 0. 001 ). (2) The patients" amount, the non - death amount, the death amount, the non - sudden cardiac death amount and the sudden cardiac death amount in pre - hospital medical care were at most in first quarter respectively. (3)In 9729 cases of pre -hospital medical care, the patients'amount of trauma was at most, in which the patients" age grades was among 21 - 50 mostly, and the others in sequence were circulatory system, nervous system respectively, in which the patients' age grades was above 51, especially above 70. (4) The death amount of circulatory system, nervous system and respiratory system ranked at the Ist,
