摘  要:目的:探讨波前像差仪与医学验光测量屈光度的一致性。方法:应用WaveScan波前像差仪和医学验光方法测定136例(267眼)近视患者的球镜、柱镜及其轴向,运用Bland-Altman法和Pearson相关分析评价两者的一致性。结果:Pearson相关分析显示,两种验光方法的球镜(r=0.9550,P<0.01)、柱镜(r=0.8812,P<0.01)和等效球镜(r=0.9549,P<0.01)均具有很高的相关性。而Bland-Altman分析表明:波前像差仪验光的球镜均值高于医学验光的球镜均值,95%一致性界限范围内波前像差仪验光与医学验光的球镜差值的最大绝对值(1.49DS)高于临床球镜误差可接受范围(-0.50~+0.75D);波前像差仪验光的柱镜均值高于医学验光的柱镜均值,一致性界限范围内波前像差仪验光与医学验光的球镜差值的最大绝对值(0.74DC)在临床柱镜误差可接受范围(-0.50~+0.75D)内;两种验光方法的柱镜轴向一致性较好。结论:波前像差仪与医学验光球镜、柱镜和等效球镜的高度相关,两方法的球镜临床一致性欠佳,但柱镜及柱镜轴向的临床一致性较好。Objective To investigate the consistency of refractive diopter measurement between the wavefront aberration analyzer and the medical optometry. Methods The wavefront aberration analyzer and the medical optometry were used to measure the refractive errors in 267 eyes of 136 myopia patients which were randomly selected from the visual science and optometry center of Guangxi. The data of sphere, cylinder and cylinder axial were used for Bland-Altman analysis and the Pearson correlation analysis. Results Pearson correlation analysis showed that sphere (r = 0.9550, P 〈 0.01), cylinder (r = 0.8812, P 〈 0.01), and spherical equivalent (r = 0.9549, P 〈 0.01) were highly correlated between these two methods. The Bland-Ahman analysis showed that the degrees of sphere and cylinder measured by Wavefront aberration analyzer were higher than those by medical optometry, 4.5% of the points was beyond the 95% limits of agreement, and the maximum absolute value of the sphere difference (1.49DS) was over the acceptable range of clinical spherical error (-0.50 - + 0.75D), and the maximum absolute value of the cylinder difference (0.74DC) was in the acceptable range of clinical cylinder error (-0.50 -- + 0.75D) for the wavefront aberration analyzer and the medical optometry within the 95% limits of agreement. The consistency rate of cylinder axial was perfect between these two methods. Conclusion The results from wavefront aberration analyzer and the medical optometry are highly correlated, the spherical degree is not good at the clinical consistency, but the cylinder degree and cylindrical axial are perfect at the clinical consistency.
