急性出血性阑尾炎散发病例分析

摘　　要：目的:调查临床上散发的急性出血性阑尾炎病例,以证明该疾病是否为一种潜在的新型疾病。方法:通过发现满足以下两个条件的病例以验证假设:①在临床病例中能发现与急性出血性阑尾炎具有相同组织学特征的病例;②这些病例的其它特征也与急性出血性阑尾炎相同。结果:755例急性单纯性阑尾炎中筛选出203例,1000例急性化脓性阑尾炎中筛选出91例具有急性出血性阑尾炎组织学特点的病例。病例的外周血白细胞计数与中性粒细胞分类计数低于急性经典阑尾炎,外周血嗜酸性粒细胞浸润升高。结论:这些病例满足上述两个条件,提示急性出血性阑尾炎可能为一种临床上常见的独立实体,如果本研究能够得到进一步证实,将有可能改变对阑尾炎的传统认识,进而改进急性出血性阑尾炎的防治方法。Objective：To survey on the sporadic cases of acute hemorrhagic appendicitis in Hubei province of China.Methods：The cases included in this survey had not only had the same histopathological features,but also the same clinical features with acute hemorrhagic appendicitis.Results：A total of 203cases from 755cases of acute simple appendicitis and 91cases from 1000cases of acute suppurative appendicitis were screened out as acute hemorrhagic appendicitis.The total leukocyte count and neutrophil count of acute hemorrhagic appendicitis patients were lower than that of classic acute appendicitis,and eosinophils count also increased in the acute hemorrhagic appendicitis cases.Conclusion：The acute hemorrhagic appendicitis may be a common entity.If this hypothesis get further verified,the traditional classic notion of appendicitis would be changed and might further lead to the changes of prevention and cure.
