肝癌相关抗原MRP-L28血清学特异性研究及其临床意义

摘　　要：目的检测肝癌相关抗原MRP-L28的肝癌血清学特异性。方法应用SEREX技术检测MRP-L28在肝癌、胃癌、肺癌、直肠癌、肝炎、肝硬化病人及正常人血清中的抗体阳性率。结果相应抗体主要见于HCC患者血清,肝癌病人血清阳性反应率为48.4%;肝炎、肝硬化病人血清阳性反应率均为6.5%;肺癌、直肠癌、胃癌的病人血清阳性反应率分别是6.5%、9.7%和3.2%;正常人血清阳性反应率为3.1%。经比较,MRP-L28抗体在肝癌血清中的阳性率最高(P<0.05);其余各组之间MRP-L28抗体阳性率无统计学意义。同时,本文还比较了MRP-L28抗体阳性率与AFP阳性率及含量、病理分级、年龄和γ-谷氨酰转肽酶(GGT)的关系,结果显示无统计学意义。使用MRPL-28诊断肝癌的灵敏度、特异性和准确度分别为48.4%、94.1%和87.6%。结论 MRP-L28在HCC检测中有一定的特异性,在肝癌血清学诊断方面具有一定的临床意义,可以考虑作为新的HCC血清学诊断标记物。【Objective】To investigate the serologic specificity of a tumor associated antigen MRP-L28 in HCC.【Methods】SEREX-technique was used to detect anti-MRP-L28 antibody in the sera of patients with HCC,lung cancer,rectal cancer,gastric cancer,hepatitis and liver cirrhosis and the sera from healthy individuals.【Results】Of thirty-one sera from HCC patients,there were fifteen showed antibody-positive responses to MRP-L28（15/31,48.45 %）.Positive rate for antibody responses to MRP-L28 was 6.5%（2/31） in the both of hepatitis and liver cirrhosis patients.Antibody-positive responses to MRP-L28 in the patients with lung cancer,rectal cancer and gastric cancer were 6.5 %（2/31）,9.7 %（3/31） and 3.2 %（1/31）,respectively.And of thirty-two sera from healthy individuals,only one showed antibody-positive response to MRP-L28（1/32,3.1 %）.There was a statistic significance between HCC and other tested groups（P 0.05）.There were no distinct statistic differences between normal individuals and non-HCC groups（P 0.05）.Furthermore,the high antibody responses to MRP-L28 in HCC sera were not related to the clinical parameters such as AFP,pathological grade,ages and GGT.The sensitivity,specificity and accuracy of MRP-L28 in the diagnosis of HCC were 48.4 %,94.1 % and 87.4 %,respectively.【Conclusion】With certain specificity in HCC,MRP-L28 may have some detected significance and be considered as one of new serological markers in diagnosis of HCC.
