摘　　要：目的：评价闭合性松解术结合解锁手法治疗膝关节骨性关节炎的有效性及安全性。方法：采用多中心、随机的临床研究，将269例患者分为试验组（133例，167膝）和对照组（136例，172膝）。试验组针对造成KoA功能障碍的高应力点用针刀进行选择性的松解，松解点包括：韧带、滑囊、关节内、神经卡压点等。对照组采用针灸。二组均结合解锁推拿法进行综合治疗。通过SF—MPQ；膝关节骨性关节炎严重性指数（ISOA）明确其治疗方法的有效性；通过安全分级的方法，评估其安全性。结果：试验组和对照组在治疗前一般情况比较无明显差异；治疗后试验组各项观察指标均优于对照组，可明显提高患者生活质量，治疗周期明显缩短，且远期疗效确切。二组的安全性无明显差异。结论：闭合性松解术结合解锁手法治疗膝关节骨性关节炎疗效确切，安全可靠，具有微创或无创以及“简、便、验、廉”等特点，是治疗膝关节骨性关节炎有效手段之一。Objective： Appraisal the effective and secure values of treating knee joint osteoarthritis（KOA） by closed relaxing operation with unlocking massage manipulation. Method： Divide the 269 patients into testing group （133 cases, 167 knees） and contrast group （136 cases, 172 knees） with the more centeral and radom clinical research method. The testing group do the selective relaxation according to the high-stress-points of causing the functional disturbance of KOA, the relaxed points include： ligament, synovial bursa, internal joint, nerve press-blocking point, etc. The contrast group adopt the acupuncture and moxibustion. The two groups all do the comprehensive treatments combining with the unlocking massage manipulation. Make sure the effectivity of treating method through the measure and analysis of SF-MPQ, KOA index （ISOA）; appraisal its security through the method of secure classification. Result： Before treating the testing group and contrast group generally have no obvious difference; after treating every index of the testing group which treated by closed relaxing operation with unlocking massage manipulation all better than the contrast group, can remarkably improve the living quality of patient, decrease the treating time, the long term curative effect is definite. The secure properties of the two groups have no notable difference. Conclusion： The curative effect that treating knee joint osteoarthritis by closed relaxing operation with unlocking massage manipulation is definite, it is secure, possess the characteristics of minimal injury or noninjury and ＂simplicity, convenience, reliable curative effect, low-price ＂, etc. It is one of the effective treating means of KOA.
