肺切除加部分左心房切除在局部晚期肺癌手术中的应用(附16例报告)

摘　　要：目前探讨左心房部分切除的肺扩大切除术治疗局部晚期肺癌的手术方法及临床疗效。方法2001年12月至2006年12月,16例肿瘤累及左心房的局部晚期肺癌患者施行肺叶或全肺切除时,同时切除部分左心房。其中左肺下叶切除6例,左全肺切除4例,右肺中下叶切除3例,右肺下叶切除3例。结果16例患者均顺利完成手术,无手术死亡及术中并发症。术后有2例发生心律失常,2例并发肺炎。平均存活时间36个月,术后1、3、5年生存率分别为75%(12/16)、43.75%(7/16)、25%(4/16)。结论肺切除加部分左心房切除的扩大切除术治疗局部晚期肺癌,术后配以放化疗,具有较好的临床疗效,能明显提高其生存质量并延长生存时间。Objective To discuss the operative method and clinical effect of pneumonectomy plus partial resection of left atrium in locally advanced lung cancer.Methods From Dec.2001 to Dec.2006,16 patients with locally advanced lung cancer involving left atrium received partial resection of left atrium plus pneumonectomy were analyzed retrospectively.In these cases,6 underwent the left lower lobe resection,4 underwent left pneumonectomy,3 underwent right and lower lobe resection,3 underwent right middle lower lobe resection.Results No operative death or intraoperative complication was found.Arrhythmia happened in 2 cases,pneumonia in 1 case.The average survival period was 36 months.The postoperative survival rates of 1,3,5-years were in 75%（12 /16）、43.75%（7 /16）、 25%（4 /16）retrospectively.Conclusion Pneumonectomy combined with partial resection of left atrium and postoperative chemotherapy have a good clinical effect,and would improve the patients＇survival quality and prolong the survival time.
