图像引导放疗同步TP方案化疗治疗局部晚期非小细胞肺癌的近期疗效观察

摘　　要：[目的]评价局部晚期非小细胞肺癌（NSCLC）接受放化疗综合治疗结果。[方法]46例局部晚期非小细胞肺癌接受图像引导放疗联合TP方案化疗模式治疗。化疗方法：国产多西紫杉醇（商品名希存,深圳万乐医药公司生产）75/m^2静滴d 1,PDD（顺铂）25 mg/m^2,静滴d 1～3。21～28 d为1个治疗周期。图像引导放疗方法：应用医科达Synergy医用直线加速器行图像引导放疗,中位放疗剂量62 Gy（55-69 Gy）。[结果]46例病人完全缓解4例,部分缓解33例,稳定6例,进展3例,总有效率81.4%（37/46）。1年生存率为71.7%（33/46）。放射性食管炎25例（54.3%）,其中Ⅰ-Ⅱ级21例（45.6%）,Ⅲ级4例（8.7%）;放射性肺炎5例（10.9%）,均为Ⅰ-Ⅱ级,经积极处理患者均可耐受。[结论]图像引导放疗联合TP方案化疗,疗效确切,副反应可以耐受,值得临床推广。[Objective]To evaluate the acute side and clinical efficacy of Docetaxel plus cisplatin combined with image guide radiotherapy for locally advanced non-small cell lung cancer.[Methods]46 patients with locally advanced NSCLC were given Docetaxel 75mg/m2 by intravenous infusion on day 1,and DDP25mg/m2 by intravenous infusion on day 1-3,at the same time,they received IGRT to total dose of 62Gy（55-69Gy）.[Results]The response rate（RR）was 81.4%.The one-year survival time was 71.7%.25 patients had radiation esophagitis,in which 21 patients had Ⅰ-Ⅱ degree,4 patients reached Ⅲ degree.5 patients had radiation pneumonitis（Ⅰ-Ⅱ degree）.[Conclusion]Docetaxel plus cisplatin（DDP）combined with IGRT for locally advanced NSCLC can achieve good response and with acceptable complications.
