腹腔镜输入段结扎胆肠袢式吻合术治疗晚期恶性阻塞性黄疸的临床效果

摘　　要：目的：探讨腹腔镜输入段结扎胆肠袢式吻合术治疗晚期恶性阻塞性黄疸的临床效果。方法：为11例晚期恶性阻塞性黄疸患者施行腹腔镜输入段结扎胆肠袢式吻合术,统计手术时间、术中出血量、术后肝功能恢复等指标。结果：11例均顺利完成手术,手术时间100-180min,术中出血10-50ml,术后3d血直接胆红素开始下降,术后肝功能明显改善。结论：腹腔镜输入段结扎胆肠袢式吻合术患者手术创伤小,术后康复快,减黄效果好,是晚期恶性阻塞性黄疸减黄的有效手段。Objective：To explore the clinical effect of laparoscopic uncut Warren choledochojejunostomy for the late malignant obstructive jaundice.Methods：Eleven patients with late malignant obstructive jaundice were treated by laparoscopic uncut Warren choledochojejunostomy.The clinical data including the operative time,the blood loss,and postoperative recovery of hepatic function were observed.Results：All operations were successful.The operative time ranged from 100min to 180min,the blood loss ranged from 10ml to 50ml,direct bilirubin in blood decreased 3 days after operation,hepatic function obviously improved after operation.Conclusions：Laparoscopic uncut Warren choledochojejunostomy has its advantage of less trauma,more rapid recovery and effective reduction of jaundice,it is an effective treatment to reduce jaundice for the late malignant obstructive jaundice.
