原发性胆囊癌患者手术前后NK细胞活性及T细胞亚群的改变

摘　　要：目的：探讨原发性胆囊癌患者手术前后机体自然免疫防御机能和后天获得性免疫机能的对比结果。方法：以１５例手术前后的原发性胆囊癌患者及２５例正常对照为研究对象，采用微量细胞毒试验－ＬＤＨ释放法及碱性磷酸酶－抗碱性磷酸酶法（ＰＡＰ），对自然杀伤细胞（ＮＫ）活性和Ｔ细胞亚群变化进行了序列测定。结果：原发性胆囊癌患者的ＮＫ细胞活性和Ｔ细胞亚群中ＣＤ３＋，ＣＤ４＋细胞均明显低于正常对照组，而ＣＤ８＋细胞增高，ＣＤ４＋／ＣＤ８＋细胞比值减小，胆囊癌术后患者几种细胞免疫功能指标在原有功能抑制基础上进一步降低，呈现出一个短暂的术后免疫抑制期，继而各项免疫指标均有不同程度的恢复，该抑制期持续时间长短与胆囊癌患者的预后密切相关。结论：胆囊癌根治术可消除这种肿瘤性免疫抑制，提高ＮＫ细胞活性和ＣＤ３＋，ＣＤ４＋细胞百分值，对原发性胆囊癌患者手术前后定期比较ＮＫ细胞活性和Ｔ细胞亚群的变化，有助于对患者的预后作出正确评估。Objective:To compare the functions of natural immunity and acquired immunity in patients with primary gallbladder carcinoma (PGC) before operation after operation.Methods:Fifteen patients with PGC and twenty-five normal persons were investigated by means of minute with those cell toxin experiment-LDH enzyme release assay and alkaline phosphoric acid enzyme-anti alkaline phosphoric acid enzymea (PAP),in order to detect changes of NK activity and T lymphocyte subsets postoperation alignmently.Results:Both the NK activity and CD 3+ and CD 4+ cell of T lymphocyte in patients with PGC are lower than those of normal persons significantly( P <0.001),While the CD 8+ cell is increased and the ratio of CD 4+ /CD 8+ is decreased,Several cell immunity function indexes were decreased furtherly and a immunity restrain period of short duration has emerged postoperatively Afterwards,each immunity index was resumed gradually,The time of restrain period was cbsely related to the prognosis of the patients with PGC.Conclusion: The radical operation of PGC can eliminate such tumor immunity restrain and enhance the NK activity and the percentage of CD 3+ and CD 4+ ,The result showed that comparing NK activity and T lymphocyte subsets before operation with those after operation in patients with PGC regularly would here contribute to evaluate prognosis correctly.
