芪黄煎剂胃癌术后早期应用对抗炎细胞因子及其mRNA表达的影响

摘　　要：目的探讨胃癌术后早期应用健脾通里中药芪黄煎剂对血清抗炎性细胞因子及其mRNA表达的影响。方法将41例需要外科手术的胃癌患者,随机分为研究组(21例)和对照组(20例)。研究组术后1 d开始,采用肠内滴注能全素联合中药芪黄煎剂;对照组仅滴注能全素。疗程均为7 d。术前1 d及术后1和7d,分别检测两组血清白细胞介素-2(interleukin-2,IL-2)、白细胞介素-4(interleukin-4,IL-4)、白细胞介素-10(interleukin-10,IL-10)水平及其mRNA表达水平。结果术后1 d,两组血清IL-2、IL-4、IL-10均显著下降(P<0.05)。术后7 d,研究组上述细胞因子水平显著上升,与本组术后1 d比较,差异有统计学意义(P<0.05);IL-2和IL-4水平显著高于对照组(P<0.05)。术后1 d,两组血清IL-2、IL-4、IL-10 mRNA表达水平显著下降(P<0.05)。术后7 d,研究组IL-2、IL-4、IL-10 mRNA表达水平显著增加,与本组术后1 d及对照组术后7 d比较,差异均有统计学意义(P<0.05)。结论胃癌术后早期小肠内滴注健脾通里中药芪黄煎剂,通过促进抗炎性细胞因子(IL-2、IL-4、IL-10)mRNA的表达,使血清抗炎性细胞因子水平回复正常,对胃癌术后机体免疫功能的恢复和炎性反应的控制具有重要作用。Objective To observe the effects of Qihuang Decoction（a traditional Chinese prescription for supplementing spleen-qi and promoting defecation） on anti-inflammatory cytokines and their mRNA expression in early postoperative patients with gastric carcinoma.Methods Forty-one gastric carcinoma patients with operation indications were randomized into study group（21 cases） and control group（20 cases）.On the first day after the operation,patients in the study group were dripped with Nutrison combined with Qihuang Decoction via the silicon catheters intubated into small intestine,while patients in the control group were dripped with Nutrison only.The treatment course was 7 d in both groups.On the first day before the operation,and on the first and seventh day after the operation,the serum levels of interleukin-2（IL-2）,interleukin-4（IL-4）,and interleukin-10（IL-10）together with their mRNA expression were assayed,respectively.Results On the first day after the operation,the serum levels of IL-2,IL-4,and IL-10 in both groups were significantly decreased（P〈0.05）.On the seventh day after the operation,the serum levels of above-mentioned cytokines were significantly increased in the study group as compared with those on the first day after the operation（P〈0.05）.The serum levels of IL-2 and IL-4 in the study group were significantly higher than those of the control group on the seventh day after the operation（P〈0.05）.On the first day after the operation,the expression levels of IL-2,IL-4,and IL-10 mRNA were statistically decreased（P〈0.05）.On the seventh day after the operation,the expression levels of IL-2,IL-4,and IL-10 mRNA were statistically increased in the study group as compared with those on the first day afterthe operation,and they were statistically higher than those in the control group on the seventh day after the operation（P〈0.05）.Conclusion Application of Qihuang Decoction in early postoperative patients with gastric carcinoma can promote the expression of anti-i
