鼻腔鼻窦恶性肿瘤临床治疗分析

摘　　要：鼻腔鼻窦恶性肿瘤约占全身肿瘤的0．5％，本文就64例鼻腔鼻窦恶性肿瘤单纯或综合治疗下的局部控制及5年生存情况进行分析。结果：本组总局部控制率为70．3％，Ⅱ、Ⅲ、Ⅳ期的控制率分别为76．5％、72．7％和70％。手术标本切缘阴性局部控制率为73．8％，而阳性则为33．3％。5年生存率为47．8％，综合治疗的5年生存率高于单一治疗，本组资料显示S＋R和A＋R＋S较高，分别为61．1％和62．0％，而S和R则较低（33．3％和28．6％，P＜0．01）。标本切缘、颈淋巴结转移及T分期，治疗方式均影响预后，综合治疗的疗效好，早期发现及较彻底切除是获得良好治疗效果的关键。Sixty-four patients with nasal cavity or paranasal sinus carcinoma weretreated in our department from 1980 to 1997. There were 43 males and 21 females,ranging in age from 18 to 75 years. Of them,the cancer in nasal cavity was 18 (28. 1 % ),maxillary sinus 34 (53. 1 % ), ethmoidal sinus 11 (17. 2%), and sphenofrontal sinus in 1(1. 6% ) respectively. The stages of them,stage 1 was 17(26. 6% ), Ⅲ 22 (34. 4%), Ⅳ21 (32. 8 % ) and others 4 (6. 3% ). The treatment methods were including surgery alone(S) in 12 (18. 8 % ),surgery+radiotherapy (S+R) in 18 (28. 1 % ),and surgery+radio-theragy+chemotherapy (S +R +A) in 21 (32. 8% ), and surgery +chemotherapy (S +A) in 6 (9. 4% ). Our results showed that the total local control rate was 70. 3% (45/64),and in stages Ⅱ, Ⅲ, Ⅳ as 76. 5% (13/17),72. 7% (16/22),and 70. 0% (14/20) respectively. The local control rate of operative speimen edges negative was 73. 8% (31/42),and in those of the edges positive was 33. 3 % (7/21 ). The total 5-year survival ratewas 47. 8% (33/64). The 5-year survival rate in group S + R and group A + R + S61. 1 % (11/18) and 62. 0% (13/21 ) respectively, which is more than that of group S(33. 3%, 4/12) and group R (28. 6%, 2/7) (P <0.01 ). The affectal factors of prognosis were inluding specimen edges,metastasis of neck lymphatic nodes,T stages and differental treatmental methods.
