纳屈酮治疗大鼠急性出血坏死性胰腺炎内毒素血症的效果

摘　　要：目的探讨纳屈酮治疗大鼠急性出血坏死性胰腺炎内毒素血症的效果．方法应用５０ｇ／Ｌ牛磺胆酸钠逆行胰胆管注射诱发大鼠急性出血坏死性胰腺炎（ＡＨＮＰ）模型，观察对照组、ＡＨＮＰ组及纳屈酮（ＮＴＸ）治疗组６，１２，２４ｈ大鼠血浆淀粉酶、内毒素（ＥＴ）及胰腺组织光镜和电镜下病理改变，并统计３组大鼠４８ｈ存活率结果ＡＨＮＰ组较对照组：于６，１２，２４ｈ时点血浆淀粉酶（２４０７±５１２，２８７２±４１３，３４１５±５９４，ｎｍｏｌ／ｓ）、ＥＴ（４５±２３，９３±１２，１６０±３５，ＥＵ／Ｌ）升高；ＮＴＸ组较ＡＨＮＰ组：血浆淀粉酶（１５３２±５６７，１８９５±５９４，２４８３±８１８，ｎｍｏｌ／ｓ）、ＥＴ（２８±３，２１±８，６９±９，ＥＵ／Ｌ）下降；而且光镜及电镜下，ＮＴＸ组胰腺腺胞及间质病理损害减轻．结论ＮＴＸ可通过降低ＡＨＮＰ大鼠血浆ＥＴ，而改善胰腺病理损害。AIM To observe the therapeutic effects of naltrexone(NTX) in plasma endotoxin in experimental acute hemorrhagic necrotizing pancreatitis of rats.METHODS A model of acute hemorrhagic necrotizingpancreatitis (AHNP) was induced by retrograde injectionof 50 g/ L sodium taurocholate into the pancreatic duat.The rats were randomly divided into three groups: controlgroup, AHNP group and NTX group. The effects of pendophin recepter blocker, naltrexone on plasma levels ofamylase and endotoxin, and changes of the pancreatichistology were examined under light and electronic microscopy and the survival rates of the three groups werecalculated.RESULTS Plasma levels of amylase (nmol/ s, 2407 512,2872 413, 3415 594) and endotoxin (EU/ L, 45 23, 93 12, 160 35) in 6, 12 and 24 h fell significantly and thedamage of pancreatic tissue was reduced.CONCLUSION NTX can decrease plasma endotoxin levelsso as to improve the damage of pancreatic tissues andlower the death rate of the AHNP rats.
