摘 要:目的:研究腺样囊性癌中缺氧诱导因子-1α(hypoxia-induciblefactor1alpha,HIF-1α)和血管内皮细胞生长因子(vasculareendothelialgrowthfactor,VEGF)表达的相关性,及其与腺样囊性癌患者临床病理和生存期之间的相关性。方法:应用免疫组化SP法,检测40例腺样囊性癌、20例多形性腺瘤和20例正常唾液腺组织中HIF-1α和VEGF的表达,并对腺样囊性癌患者进行随访观察。采用SPSS13.0软件包进行统计学分析,生存分析采用Kaplan-Meier法,预后分析采用Cox模型。结果:HIF-1α和VEGF的阳性表达率和微血管密度计数在正常唾液腺组织、多形性腺瘤、腺样囊性癌中依次增加,组间差异具有统计学意义(P<0.05);MVD在腺样囊性癌中计数均随着HIF-1α和VEGF表达的增强而升高(P<0.05);HIF-1α和VEGF两者在腺样囊性癌之间也具有相关性(P<0.05);HIF-1α和VEGF的表达与腺样囊性癌的生存期、肿瘤的转移有关,组间差异具有统计学意义(P<0.05),但与腺样囊性癌肿瘤大小、神经侵犯、病理分型、淋巴结转移间的差异无统计学意义。结论:HIF-1α和VEGF的表达与腺样囊性癌的转移和患者短生存期密切相关。PURPOSE:To investigate the expression of hypoxia-inducible factor 1 alpha and vascular endothelial growth factor expression,and their relations to angiogenesis and the clinicopathological characters in salivary adenoid cystic carcinoma.METHODS:SP immunochemistry was applied to detect the expression of HIF-1α and VEGF;Microvessel density(MVD) was counted by anti-CD34 antibody in 40 cases of salivary adenoid cystic carcinoma(ACC),20 cases of pleomorphic adenoma(PA),20 cases of normal salivary gland tissue.40 ACC patients were followed up,nonparametric tests were performed for the comparison of HIF-1α and VEGF expression between two independent groups.Survival analysis were performed to find prognostic significance of HIF-1α and VEGF expression using SPSS13.0 software package.RESULTS:Positive expression of HIF-1α and VEGF enhanced gradually in normal salivary gland tissue,PA and ACC(P0.05).MVD tended to depend on HIF-1α and VEGF expression levels in ACC(P0.05).Expression of HIF-1α waspositively correlated to VEGF in ACC(P0.05).HIF-1α and VEGF expression was significantly associated with tumor survival and metastasis,but did not correlate with T stage,N stage,neural invasion and pathological grade.CONCLUSION:The expression of HIF-1α and VEGF may play an important role in the short survival time and metastasis of ACC.
