血清CA125、VEGF表达水平与卵巢癌分期、疗效及生存时间的临床分析

摘　　要：目的：探讨血清CA125、VEGF表达水平与卵巢癌分期、疗效及生存时间的关系。方法：采用回顾性对1998年1月～2003年1月诊治的临床资料完整的卵巢癌78例治疗前后血清CA125、VEGF水平与卵巢癌病理、分期、疗效及生存时间临床资料进行分析。结果：卵巢浆液癌CA125、VEGF阳性率分别为83．4％、88．9％，与其它病理类型差异无统计学意义（P〉0．05）；肿瘤细胞减灭术后，残留〈2em者CA125、VEGF阳性率分别为37．5％、25．0％；术后化疗CA125、VEGF阳性率分别为32．3％、22．6％（P〈0．05）；血清CA125（＋）、VEGF（＋）中位生存时间19．8个月；血清CA125（-）、VEGF（-）中位生存时间54．2个月。结论：血清CA125、VEGF表达水平与卵巢癌病理类型无关；血清CA125、VEGF高水平表达，提示临床分期晚；肿瘤细胞减灭术应达到肉眼无残留，术后尽快规范化疗，尽早使血清CA125、VEGF降至正常水平，延长生存时间。Objective： To explore the relationship between expression levels of serum CA125, VEGF and staging, curative effect and survival time of patients with ovarian carcinoma. Methods： 78 patients with ovarian carcinoma having complete clinical data who were diagnosed and treated in the hospital from January 1998 to January 2003 were analyzed retrospectively; the expression levels of serum CA125, VEGF and staging, curative effect and survival time of patients with ovarian carcinoma were analyzed. Results： The positive rates of CA125 and VEGF of ovarian serous carcinoma were 83.4% and 88.9%, respectively, there was no significant difference between ovarian serous carcinoma and other pathological types （P 〉 0.05 ） ; after cytoreductive surgery, the positive rates of CA125 and VEGF of residual 〈 2 cm were 37.5% and 25.0%, respectively; for the patients receiving postoperative chemotherapy, the positive rates of CA125 and VEGF were 32. 3% and 22. 6% , respectively （ P 〈 0. 05 ） ; the median survival time for patients with positive CA125 and VEGF was 19- 8 months ; the median survival time for patients with negative CA125 and VEGF was 54.2 months. Conclusion： There is no correlation between expres- sion levels of serum CA125, VEGF and pathological types of ovarian carcinoma; the high expression levels of CA125 and VEGF mean late clinical stages ; cytoreduetive surgery should be up to no residue by naked eye, postoperative standard chemotherapy as soon as possible and reducing serum levels of CA125 and VEGF to normal levels may prolong survival time.
