颌面部软组织损伤并腮腺损伤31例治疗体会

摘　　要：目的探讨颌面部软组织损伤并腮腺损伤治疗体会。方法回顾2008年1月至2010年2月收治的颌面部软组织损伤并腮腺损伤31例临床资料进行总结。结果本组急性21例颌面部软组织损伤并腮腺损伤患者通过对腮腺或其主导管损伤进行积极处理,21例患者在10~20d,平均16d,伤口Ⅰ期愈合,没有出现1例腮瘘、腮腺导管瘘;急性损伤中进行面神经吻合的4例,面神经8~10个月功能基本恢复正常3例,另1例于1年后恢复。陈旧性损伤10例中腮腺腺体瘘及导管瘘的患者经积极治疗均全部治愈,2例面神经吻合神经移植,1年后面神经功能基本恢复。结论颌面部软组织损伤并腮腺损伤病例应在创伤早期进行细致的清创,及早发现腮腺损伤,进行急性期合理处理,减少并发症发生。对陈旧性损伤应判断损伤情况,采取不同的积极处理方法促进腮腺导管瘘、腺体瘘及面神经损伤恢复。Objective To explore the maxillofacial soft tissue injury and treatment of parotid gland injury.Methods Analyze clinical data of January 2008-2,010 admitted in February maxillofacial soft tissue injuries and 31 cases of parotid gland injury.Results 21 cases of acute soft tissue injury and facial injury patients with parotid gland or through the main duct injury in active treatment,21 patients in 10~20 days,an average of 16 days,wound healing Ⅰ,no one case of mumps fistula,parotid duct fistula;acute injury to facial nerve anastomosis in 4 patients,8~10 months,the facial nerve function returned to normal in 3 cases,and 1 case 1 year later relapsed.Old injury in 10 cases of parotid gland fistula and duct fistula patients after active treatment were cured,2 cases of facial nerve anastomosis,nerve function 1 year later recovered.Conclusion The facial soft tissue injury and parotid gland injury cases should be careful in the early wound debridement,early detection of parotid gland injury,and reasonable treatment for acute and reduce complications.To the old injury should determine damage,take a different approach to promote the positive parotid duct fistula,fistula gland and facial nerve injury recovery.
