奥曲肽治疗重症急性胰腺炎患者CRP及DD的变化

摘　　要：目的探讨奥曲肽对重症急性胰腺炎(SAP)患者C-反应蛋白及D-二聚体的影响。方法 98例非手术治疗SAP患者,分为对照组50例予常规治疗;奥曲肽治疗组48例在对照组治疗基础上加奥曲肽0.6 mg静滴,每12 h 1次。观察治疗前后临床疗效、并发症的发生及C-反应蛋白、D-二聚体的变化。结果奥曲肽治疗组较对照组临床疗效提高,并发症、病死率降低,C-反应蛋白降低,D-二聚体降低,全血黏度、血浆比黏度、血小板黏附率、红细胞电泳时间缩短。结论奥曲肽能降低C-反应蛋白及D-二聚体,改善SAP的血液流变学,减轻胰及胰外器官损害和并发症的发生。Objective To observe the effect of Gabexate on C-reactive protein and D-dimer in severe acute pancreatitis.Methods 98 Non-surgical treatment patients with SAP were divided into two groups randomly.50 cases in control group treated by conventional therapy.Gabexate treatment group had 48 cases,which were treated with gabexate by intravenous infusion to maintain 12 hours on the basis of the control group.The changes of clinical curative effect,C-reactive protein and D-dimer were observed before and after treatment.Results Compared with control group,the clinical curative effect of Gabexate treatment increased,C-reactive protein,D-dimer,case fatality ratio and complication decreased.The whole blood viscocity,blood plasma specific viscosity,platelet sticky rate and erythrocyte electrophoresis improved.Conclusion Gabexate can degrade C-reactive protein and D-dimer,improve the blood rheology of SAP and lessen injury of pancreas and outside pancreatic organs,and can decrease complications.
