外周血和痰中hnRNP A2/B1检测对原发性肺癌的诊断价值

摘　　要：目的探讨外周血和痰中不均一核糖核蛋白(hnRNP A2/B1)检测对原发性肺癌的诊断价值。方法采用免疫组化SP法检测62例原发性肺癌患者(病例组)和62例良性肺部疾病患者(对照组)外周血和痰中hnRNP A2/B1阳性情况。结果病例组、对照组痰中hnRNP A2/B1阳性率分别为90.3%、16.1%,2组比较差异有统计学意义(P<0.05);病例组、对照组血中hnRNP A2/B1阳性率分别为41.9%、29.0%,2组比较差异无统计学意义(P>0.05)。hnRNP A2/B1敏感性、准确度、阳性预测值、阴性预测值:病例组痰脱落细胞分别为90.3%、87.1%、85.5%、88.7%,病例组外周血分别为41.9%、56.5%、59.7%、54.8%,病例组痰脱落细胞hnRNP A2/B1敏感性、准确度、阳性预测值、阴性预测值均明显高于外周血(均P<0.05)。结论痰脱落细胞hnRNP A2/B1检测对原发性肺癌有一定的诊断价值。Objective To explore the diagnostic value of the detection of heterogenous nuclear ribonudeopmtein A2/B1（hnRNP A2/B1） in peripheral blood and sputum in patients with primary lung cancer.Methods Sixty-two patients with primary lung cancer（case group） and 62 patients with benign lung disease（control group） were selected and the expression of hnRNP A2/B1 in peripheral blood and sputum was detected using immunohistochemistry SP method.Results The positive rate of sputum hnRNP A2/B1 in case group（90.3%） was significantly higher than that in control group（16.1%）（P〈0.05）.There were no significant differences in the positive rate of peripheral blood hnRNP A2/B1 between case group and control group（41.9% and 29.0%,respectively）（P〉0.05）.In case group,sensitivity,accuracy,positive predictive value and negative predictive value were 90.3%,87.1%,85.5% and 88.7% in sputum,respectively,and 41.9%,56.5%,59.7% and 54.8% in peripheral blood,respectively.There were significant differences between sputum and peripheral blood（all P〈0.05）.Conclusion Detection of hnRNP A2/B1 in sputum has diagnostic value in patients with primary lung cancer.
