摘 要:目的探讨首发精神分裂症患者是否存在自由基代谢异常及氧化应激损伤。方法纳入符合美国精神障碍诊断与统计手册第4版(DSM-IV)诊断标准的首发精神分裂症住院患者78例,以阳性和阴性症状量表(positive and negative syndrome scale,PANSS)评定其精神症状;检测血清总超氧化物歧化酶(T-SOD)、铜锌超氧化物歧化酶(Cu-ZnSOD)、S100B的含量。另选87名正常人作对照。结果患者组T-SOD活性、Cu-ZnSOD活性、S100B蛋白含量高于正常对照组(t=3.12,P<0.05;t=5.62,P<0.05;Z=-2.43,P<0.05)。进一步分层分析,男性患者组T-SOD、Cu-ZnSOD高于男性对照组(P<0.05),女性患者组T-SOD、S100B高于女性对照组(P<0.05)。在对照组中男性组Cu-ZnSOD低于女性组(P<0.05)。精神病家族史阳性患者组T-SOD高于阴性组(P<0.05)。患者组中男性吸烟组的T-SOD、Cu-ZnSOD低于男性非吸烟组(P<0.05)。相关分析显示,患者组的住院次数与S100B呈正相关(r=0.36,P<0.05);PANSS的阳性症状分、阴性症状分、总分分别与Cu-ZnSOD负相关(r为-0.28、-0.27、-0.31,P均小于0.05)。结论首发精神分裂症患者存在着自由基代谢异常且与精神症状相关,低剂量尼古丁可能可抑制自由基的产生。Objective To investigate the abnormalities of free radical metabolism and Oxidative stress in first-episode schizophrenic patients. Methods Seventy eight inpatients with first-episode schizophrenia diagnosed by DSM-IV were evaluated using the Positive and Negative Syndrome Scale(PANSS). Total superoxide dismutase (T-SOD) activity, cuprozinc-superoxide dismutase (Cu-ZnSOD) activity, and S100B levels were detected in both schizophrenic patients and normal controls. Results Serum T-SOD, Cu-ZnSOD activity and S100B levels were markedly higher in patient group compared with normal controls(t=3.21, P〈0.05; t=5.23, P〈0.05; t=2.25, P〈0.05). Serum T-SOD, Cu-ZnSOD activity and S100B levels were significantly different between different gender groups (F=3.59, P=0.01; F=5.44,P=0.001, Z=-2.63, P〈0.05). Cu-ZnSOD activity were significantly lower in male controls than in female controls (P〈0.05). T-SOD, and Cu-ZnSOD activity in male patients were significantly increased compared with male controls (P〈0.05).T-SOD activity and S100B levels is higher in female patients than in female controls (P〈0.05). T-SOD activity were markedly increased in patients with family history of mental disorder compared with patients with no family history(P〈0.05). T-SOD,Cu-ZnSOD activity were significantly lower in male smoking patients than male non-smoking ones(P〈0.05). The length of hospital stay was positively correlated with S100B levels (r=0.36, P〈0.05).The positive symptom subscore, the negative symptom subscore and total score of PANSS were negatively correlated with Cu-ZnSOD activity (r=-0.28,P〈0.05; r=-0.27,P〈0.05; r=-0.31, P〈0.5). Conclusions Patients with first-episode schizophrenia have abnormal free radical metabolism and this abnormal free radical metabolism is positively correlated with their psychiatric symptoms. Furthermore, low dose of nicotine may inhibit free radical formation.
