摘 要:目的:探讨RhoC在葡萄胎、侵袭性葡萄胎、绒癌中的表达及与葡萄胎恶变的相关性。方法:应用免疫组化的方法检测25例未恶变的葡萄胎、15例恶变倾向的葡萄胎、10例侵袭性葡萄胎及15例绒癌组织中RhoC的表达情况,采用化学发光法测定了40例葡萄胎患者288次血人绒毛膜促性腺激素(HCG),并结合临床资料进行分析。结果:RhoC在侵袭性葡萄胎及绒癌中表达明显高于葡萄胎(P<0.01);恶变倾向的葡萄胎中RhoC表达也明显高于未恶变的葡萄胎(P<0.01)。葡萄胎恶变与RhoC的表达、血HCG值及卵巢黄素化囊肿呈正相关(P<0.01、P<0.01、P<0.05)。9例血HCG持续阳性和6例血HCG降而复升的葡萄胎患者发生恶变,且9例血HCG持续阳性的患者葡萄胎中RhoC表达显著高于其他患者。结论:RhoC可能与滋养细胞的侵袭过程及程度有关,RhoC的表达上调可能是滋养细胞恶性转化的早期事件,检测葡萄胎组织中RhoC的蛋白表达,可能成为预测葡萄胎恶变的新指标,为临床预防性化疗提供依据。Objective:To explore the expressions of RhoC in hydatidiform mole, invasive hydatidiform mole and choriocarcinoma and its correlation with carcinomatous change of hydatidiform mole.Methods:Immunohistochemical method was used to detect the expression levels of RhoC in 25 cases with non-malignant hydatidiform mole, 15 cases with malignant tendency of hydatidiform mole, 10 cases with invasive hydatidiform mole and 15 cases with choriocarcinoma, and chemiluminescence method was used to detect the levels of human chorionic gonadotropin (HCG) in blood for 288 times among 40 cases with hydatidiform mole, then the clinical data were analyzed.Results:The expression levels of RhoC in invasive hydatidiform mole and choriocarcinoma were significantly higher than that in hydatidiform mole (P〈0.01).The expression level of RhoC in cases with malignant tendency of hydatidiform mole was significantly higher than that in cases with non-malignant hydatidiform mole (P〈0.01).There was a positive correlation between carcinomatous change of hydatidiform mole and RhoC expression, blood HCG level and ovarian lutein cyst (P〈0.01, P〈0.01, P〈0.05).Carcinomatous change of hydatidiform mole occurred in 9 cases with continuous positive HCG and 6 cases whose blood HCG level increased after decreasing, and the expression level of RhoC in 9 cases with continuous positive HCG was significantly higher than those in other cases.Conclusion:RhoC may be related to invasion and degree of trophoblast cells, up-regulation of RhoC may be a early event of malignant transformation of trophoblast cells, detection of RhoC protein in hydatidiform mole tissues may become a new index of predicting malignant transformation of hydatidiform mole, which provides a basis for preventive chemotherapy.
