MMP-2、MMP-9和VEGF与乳腺癌侵袭转移关系的研究

摘　　要：目的 研究血管内皮细胞生长因子（VEGF）,基质金属蛋白酶2、9（MMP-2、MMP-9）在乳腺癌组织中的表达以及与乳腺癌侵袭转移之间的关系.方法 应用免疫组织化学SP法检测20例乳腺纤维腺瘤和12例正常乳腺组织及38例乳腺癌组织中VEGF、MMP-2和MMP-9的表达.结果 乳腺癌组织中VEGF、MMP-2和MMP-9的高表达率（63.2%,78.95%,71.1%）高于正常乳腺组织和乳腺纤维腺瘤组织（25.0%,30.0%,33.3%,35.0%,41.7%,45.0%,P＜0.05）.VEGF,MMP-2,MMP-9蛋白表达与乳腺癌患者的年龄、肿瘤大小、ER及PR无关（P＞0.05）;有淋巴结转移组乳腺癌组织中VEGF、MMP-2和MMP-9的高表达率[80.0%、85.0%,85.0%]高于无淋巴结转移组（44.4%、55.6%,55.6%,P＜0.05）,与组织学分级有关（P＜0.05）.乳腺癌组织中三种蛋白VEGF、MMP-2和MMP-9之间的表达呈正相关（rs=0.541,0.511,0.502,P＜0.01）.结论 VEGF、MMP-2和MMP-9在乳腺癌中有协同作用,促进肿瘤血管形成、细胞外基质的降解和癌细胞的侵袭转移.VEGF、MMP-2及MMP-9的高表达与乳腺癌的侵袭和转移密切相关,有可能成为判断乳腺癌侵袭和转移的有价值的指标.Objective To study the expression of VEGF, MMP-2 and MMP-9 in benign and malignant lesions of breast and their effects on breast carcinoma＇s invasiveness and metastasis. Methods VEGF,MMP-2 and MMP-9 expression was detected by SP method in 20 breast fibroadenoma and 12 normal breast specimens taken from non-cancerous regions adjacent to breast cancer tissue and 38 breast carcinomas. Results The expression rate of VEGF, MMP-2 and MMP-9 in breast carcinoma was remarkably higher than those in normal breast tissue and breast fibrogdenoma （63.2% vs 25.0% and 30. 0% ,78. 95% vs 33.3% and 35.0% and 71.1% vs 41.7% and 45.0% ,respectively; P ＜0.05）. There was a correlation between VEGF, MMP-2 and MMP-9 expression and nodal metastasis （ P ＜0.05）. High expression of VEGF, MMP-2 and MMP-9 was correlated with pathological grade （ P ＜ 0.05, respectively）. The overexpression of VEGF was related with those of MMP-2 and MMP-9 in breast cancer（ VEGF and MMP-2：rs =0.541, P ＜0.01 ;VEGF and M M P-9： rs = 0.11, P ＜ 0.01; MMP-2and M MP-9： rs= 0P ＜ 0.01 ）. Conclusion VEGF,MMP-2 and MMP-9 overexpression is strongly related to the invasiveness and metastasis of breast carcinoma, which might be valuable in evaluating tumor invasion and metastasis.
