手法小切口白内障手术摘除黑核白内障的评估

摘　　要：【目的】评估对黑核白内障患者行手法小切口白内障摘除术(MSICS)的安全性和视力预后。【方法】对31名黑核白内障患者,实行手法小切口白术内障摘除术,并对晶状体核的大小进行测量。术后进行随访。【结果】31名黑核白内障患者31只眼行MSICS术。术后长期随访17人。31名患者的平均年龄为67.2(S=9.5)岁(范围49～85岁),其中女性占63.8%,术后长期随诊患者复查时间中位数为11个月(4～21个月)。术后第1天,UCVA≥0.30者70.96%。17名患者术后复查有16例(94.12%)BCVA≥0.3。有5例被发现于切口处轻度角膜水肿,前房反应轻微。没发现术后渗漏、浅前房及前房出血病例。以钢尺测量娩出后的晶状体核,其直径中位数为8.7mm(范围8.1～9.5mm)。以患者对侧未手术眼做对照,黑核组角膜散光为1.27m-1高于对照组角膜散光为0.78m-1(P﹤0.05)。【结论】手法小切口白内障摘除术用于治疗黑核白内障是安全有效的。【Objectives】 To assess the visual acuity and safety of black cataract using manual small incision cataract surgery （MSICS） in rural southern China. 【Methods】 MSICS was performed in 31 cases of black cataract. The diameter of the nucleus was measured immediately after surgery. Visual acuity and corneal astigmatism were assessed postoperatively. 【Results】 Thirty-one cases of black cataract patients were assessed postoperatively. Of these,17 cases came back to be further studied. The mean age was 67.2 ± 9.5 years and female accounted for 63.8%. Their uncorrected visual acuity UCVA ≥ 0.30 at the first post-operative day was 70.96%. Mild corneal edema was found near the corneal-scleral tunnel incision in 5 cases,which was completely withdrawn after 1 week in 3 patients examined. No case of shallow anterior chamber or hyphema was identified. Based on pin-hole visual acuity at follow-up,the prevalence of good visual acuity BCVA ≥ 0.30 was 94.12%. No case of shallow anterior chamber or hyphema was identified. The diameter of the nucleus was 8.7 mm （8.1 -9.5 mm）. Corneal astigmatism in black cataract group was higher than that of the contralateral unoperateded eye group significantly. 【Conclusion】 MSICS is safe and effective for patients with black cataract.
