广州地区某眼科医院2004-2008年眼部真菌感染病原学分析

摘　　要：【目的】分析眼部真菌感染的病原菌分布及变迁。【方法】收集中山大学中山眼科中心2004-2008年2653例眼部感染患者真菌培养的微生物实验室检验资料。对真菌培养的阳性率、菌属的分布进行回顾性分析。【结果】2653份标本中共培养真菌563株,阳性率为21.22%;其中角膜标本阳性率最高,房水标本阳性率最低。在所有菌属中,镰刀菌属最多,共184株(占32.68%),其次为曲霉菌属139株(占24.69%)及蠕孢霉属94株(占17.70%);其中来自角结膜的真菌497株,以镰刀菌属居多(占36.22%);来自玻璃体、房水的真菌61株,以曲霉菌属居多(占32.79%)。【结论】镰刀菌属和曲霉菌属是中山眼科中心眼部真菌感染的主要致病菌,且不同部位主要致病菌不同,角结膜感染主要致病菌为镰刀菌属,内眼感染主要致病菌为曲霉菌属。【Objective】 To analyze the distribution and shifting trend of pathogens causing fungal ocular infection in Zhongshan Ophthalmic Center. 【Methods】 A total of 2 653 specimens from the patients with ocular infection from January 2004 to December 2008 were collected. Fungal culture-positive rate and the genus distribution of positive cultures were analyzed retrospectively. 【Results】 Out of 2 653 specimens,563 were positive outcome with the culture-positive rate 21.22%. The specimens from cornea showed highest culture-positive rate,and those from aqueous humor showed lowest culture-positive rate. Fusarium Sp. was the most common pathogen （32.68% ）,followed by Aspergillus Sp. （24.69% ） and Helminthosporium Sp. （17.70% ）. The number of positive culture from conjunctiva and cornea was 497,in which the dominating pathogen was Fusarium Sp. （36.22%）. The number of positive culture from vitreous body and aqueous humor was 61,in which the dominating pathogen was Aspergillus Sp. （34.43%）. 【Conclusion】 Fusarium Sp. and Aspergillus Sp. are the common pathogens of fungal ocular infection in Zhongshan Ophthalmic Center. The pathogenic fungus differs in different parts of the eyes. Furasium sp. is the dominating pathogenic fungus in conjunctivitis and keratitis,and Aspergillus Sp. is the dominating pathogenic fungus in endophthalmitis.
