透明角膜切口超声乳化术与角膜内皮细胞损伤因素分析

摘　　要：目的探讨透明角膜切口超声乳化术术中各相关参数对角膜内皮细胞损伤的影响。方法 31例白内障患者共33只眼行透明角膜切口超声乳化术联合人工晶体植入术,术前评价患者术眼晶体核硬度、测量前房深度、角膜中央细胞密度,术中记录超乳时间(UST)、累积释放能量(CDE),分别于术后1周、1个月、3个月测量角膜中央角膜细胞密度,进行统计学分析。结果术前与术后1周、1个月、3个月裸眼视力、角膜内皮细胞数差异均有统计学意义(P<0.05),且术后1周与3个月裸眼视力比较差异有统计学意义(P<0.01)。术后1周、1个月、3个月角膜内皮细胞丢失率均值分别为:11.37%、14.69%、16.05%。结论透明角膜切口超乳术中角膜内皮细胞损伤的最主要因素与其他术式一致为晶体核硬度。Objective To explore the association of parameters in the clear corneal incision phacoemulsification and corneal endothelial cells injury. Methods 31 cases of cataract patients ,33 eyes who had clear corneal incision phacoemulsification and intraocular lens implantation surgery were observed. Patients undergoing eye＇visual acuity, crystal nuclear hardness,anterior chamber depth, central corneal cell density were measured pre-operatively. Ultrasound time （UST） ,cumulative dissipated energy（CDE）were recorded during operation. The central corneal endothelial cell density, visual acuity was calculated in 1 week, 1 month,3 month after operation. The data were analyzed statistically by SPSS 13.0. Results The visual acuity, central corneal endothelial cell density in 1 week, 1 month and 3 month after operation were significantly different with preoperative data, also in 1month and 3 month after operation. The ratio of corneal endothelial cell loss in 1 week, 1 month,3 month after operation was 11.37%, 14. 69% and 16.05% respectively. Conclusion Crystal nuclear hardness is the most important relative factor of the corneal endothelial cell loss in the clear corneal incision phacoemulsification.
