针刺手三里穴对肱桡肌肌张力的影响

摘　　要：目的观察针刺对中风患者和健康人肌张力的改变、患者依从性和不良反应;研究针刺对中风患者和健康人的肱桡肌肌张力的影响。方法将符合纳入标准的40例健康人设为健康组,20例经常锻炼的强壮健康人设为强壮组,20例中风患者设为中风组,分别运用针刺治疗,以肱桡肌静息及最大自主收缩两种状态下的肌张力为观察指标,通过医学统计学分析,比较针刺前后肌张力改变情况的差异。结果强壮组较健康组在针刺后15 min、30 min肌张力的差异具有统计学意义(P<0.05);中风组在针刺结束后15 min及30 min静息状态肌张力及最大自主收缩状态(MVC)肌张力与针刺前相比,差异均具有统计学意义(P<0.05),其中针刺结束后15 min及30 min静息状态肌张力与针刺前相比,差异具有统计学意义(P<0.01);中风组针刺后5 min、15 min、30 min静息状态肌张力与针刺前比较差异有统计学意义(P<0.01);针刺后5 min最大自主收缩状态肌张力与针刺前比较差异无统计学意义,而针刺后15 min、30 min与针刺前比较差异有统计学意义(P<0.05)。结论强壮组针刺后比健康组肌张力改变明显。在静息状态及最大自主收缩状态下针刺对中风患者的肱桡肌肌张力改变有显著的影响,其中静息状态下的改变更为显著。针刺可明显降低中风患者肌张力,且肌张力变化有一定时间规律。Objective To investigate the effect of acupuncture on brachioradial muscle tension in stroke patients and healthy persons by their post-acupuncture changes in muscular tension and the patient＇s compliance and adverse reactions.Methods Satisfying the inclusion criteria,40 healthy persons constituted a healthy group;20 strong healthy persons who often took exercise,a strong group;20 stroke patients,a stroke group.They were separately given acupuncture treatment.Brachioradial muscle tensions under the static state and the maximal voluntary contraction were used as observation indices.Pre-/post-acupuncture muscular tensions were compared by a medical statistical analysis.Results There was a statistically significant difference in muscular tension between strong and healthy groups at 15 and 30 min after the completion of acupuncture（P0.05）.There were statistically significant differences in muscular tensions under the static state and the maximal voluntary contraction（MVC） between before acupuncture and at both 15 and 30 min after the completion of it in the stroke group（P0.05）.Of them,there was a statistically significant difference in muscular tension under the static state before acupuncture and at 15 and 30 min after the completion of it（P0.01）.In the stroke group,there was a statistically significant difference in muscular tension under the static state before acupuncture and at 5,15 and 30 min after the completion of it（P0.01）,and there was a statistically significant difference in muscular tension under the maximal voluntary contraction between before acupuncture and at 15 and 30 min after the completion of it（P0.05） and no between before acupuncture and at 5 min after the completion of it.Conclusion After acupuncture,muscular tension changed more in the strong group than in the healthy group.Acupuncture significantly affects brachioradial muscle tensions under the static state and the maximal voluntary contraction in stroke patients.The effect is more significant under the static stat
