白内障超声乳化吸出术对泪膜及结膜上皮细胞的影响

摘　　要：目的探讨白内障超声乳化吸出联合人工晶状体植入术对泪膜及结膜上皮细胞的影响。方法年龄相关性白内障31例（38眼）。年龄54-76岁。男13例（15眼）,女18例（23眼）。行超声乳化吸出术联合后房折叠型人工晶状体植入术。分别在术前和术后第1、7、30 d测定泪膜破裂时间（BUT）、Sch irm erⅠ试验（SIT）及角膜荧光素（F l）染色,并应用免疫印迹细胞化学染色方法检测结膜上皮细胞凋亡诱发因子bax和抑制因子bc l-2的表达。结果术后第1、7天BUT值缩短、F l染色增强,二者分别与术前比较差异均有统计学意义（P〈0.05）,第30天时二者与术前比较差异无统计学意义（P〉0.05）。术后第1天SIT与术前比较差异有统计学意义（P〈0.05）,而第7、30 d时与术前比较差异无统计学意义（P〉0.05）。术后第1、7天结膜上皮细胞中凋亡诱发因子bax的表达增强、凋亡抑制因子bc l-2的表达降低,与术前比较差异有统计学意义（均P〈0.05）,而第30天时两者与术前比较差异无统计学意义（P〉0.05）。结论白内障超声乳化吸出联合人工晶状体植入术对术眼结膜上皮细胞和泪膜功能有短期的损伤,部分患者引起干眼。Objective To investigate the effects of phacoemulsification cataract extraction surgery on the tear film and conjunctival epithelial cells. Methods 31 patients（38 eyes） with cataract selected randomly to detect the function of tear film including the tear film break-up time（BUT）,schirmer I-test and corneal staining on the 1st,7th,and 30th day after the phacoemulsification extraction surgery and before the surgery,respectively.Immunohistochemical method was used to detect the expression of bac and bcl-2 in conjunctival epithelial cells. Results The BUT of the patients decreased on the 1st and 7th day after the surgery compared with the preeperative one（P0.05）.The corneal staining increased significantly on the 1st and 7th day after the surgery compared with the preeperative one.（P0.05）.while the BUT and corneal staining recovered to preoperative level on the 30th day（P0.05）.The schirmer-I of the patients increased on the 1st day after the surgery compared with the preeperative one（P0.05）.And the schirmer-I on the 7st,30th day after surgery was not obviously different with the preoperative level.The expression of bac in conjunctival epithelial cells enhanced on the 1st and 7th day after surgery（P0.05）,the expression of bcl2 in conjunctival epithelial cells decreased on the 1st and 7st day after surgery（P0.05）.while the expression of bac and bcl-2 recovered to preoperative level on the 30th day（P0.05）.Conclusion Phacoemulsification cataract extraction surgery can injury the tear film and conjunctival epithelial cells in a short time.
