幕上脑出血急性期FIB水平及其影响因素的Logistic回归分析

摘　　要：目的:观察自发性幕上脑出血急性期血浆纤维蛋白原(Plasmafibrinogen,FIB)水平及其影响因素。方法:153例急性脑出血患者,在入院或次晨测量FIB,并全面搜集可能与其相关的其他临床指标;FIB水平的分界值根据FIB水平正常参考值高限确定,FIB>4.00g/L定义为高FIB水平,将可能影响FIB的20个因素为自变量,FIB水平为应变量作单因素和多因素Lo-gistic回归分析,了解影响中到大量自发性幕上脑出血急性期FIB的因素。结果:FIB>4.00g/L者54例(36.5%);FIB≤4.00g/L者94例(63.5%)。性别、入院NIHSS评分、PLT这3个因素是中到大量自发性幕上脑出血急性期FIB的独立影响因素。(性别:OR=3.916,95%CI 1.612～9.516;NIHSS评分:OR=2.622,95%CI 1.560～4.408;PLT:OR=1.007,95%CI1.001～1.012)。结论:性别、入院NIHSS评分、PLT是中到大量自发性幕上脑出血急性期FIB水平的独立影响因素。Objective To observe the level of plasma fibrinogen（FIB） at acute phase in medium to large spontaneous supratentorial intracerebral haemorrhages（ICH） and analyze the impact factors associated with FIB.Methods 153 patients with spontaneous supratentorial intracerebral haemorrhages were included in this study.We measured their plasma FIB at next morning and obtained the other clinical data associated with the prognosis of ICH routinely.According to the higher limit of the plasma FIB level,we defined plasma FIB4.00g /L as the elevated level.Univariate and multiple logistic regression were used respectively to analyze the correlativity of the elevated plasma FIB with related 20 factors.Results The percentage of supratentorial ICH patients with plasma FIB4.0g/L was 36.5% and plasma FIB≤4.00g/L was 63.52%.The independent impact factors of plasma FIB were sex,scores of National Institutes of Health Stroke Scale（NIHSS）,and platelet（PLT）.（OR=3.916,95%CI 1.612～9.516,OR=2.622,95%CI 1.560～4.408,OR=1.007,95%CI 1.001～1.012,respectively）.Conclusions The independent impact factors of plasma FIB were sex,scores of National Institutes of Health Stroke Scale,and platelet at acute phase in medium to large spontaneous supratentorial intracerebral haemorrhages.
