闽东地区畲族人群结直肠癌发病危险因素病例对照研究

摘　　要：目的探讨闽东地区畲族人群结直肠癌发病的危险因素,以便采取科学预防措施,控制结直肠癌的发病。方法选择闽东地区畲族人群中现正存活的结肠癌患者23例,直肠癌患者68例,采用1∶1配对的病例对照研究方法,选择91名与病例匹配的同年龄、同性别、同居住地的非结直肠癌健康者,以条件Logistic回归法分析结直肠癌发病的影响因素。结果多因素Logistic回归分析结果显示:食用肥肉(OR=2.988)、经常食用火锅(OR=4.233)、食用腌制食品(OR=8.978)、情绪自我调节能力差(OR=7.623)、经常使用化学肥料(OR=2.342)、结直肠癌家族史(OR=10.729)、结直肠疾病史(OR=5.521)与结直肠癌发病呈正相关(P<0.01);而饮茶与结直肠癌发病呈负相关(OR=0.201,P<0.01)。结论食用肥肉、经常食用火锅、食用腌制食品、情绪自我调节能力差、经常使用化学肥料、结直肠癌家族史、结直肠疾病史是闽东畲族人群结直肠癌发病的危险因素,而饮茶是结直肠癌发病的保护因素。Objective To explore risk factors related to colorectal cancer and provide science basis for related intervention and preventive measures.Methods A 1∶1 matched case-control study was carried out among population of She Minority in east Fujian including 23 colon cancer cases and 68 rectal cancer cases.Data were analyzed with conditional Logistic regression.Results Analysis indicated that risk factors which were positively related to colorectal cancer included the followings： having fat（OR=2.988）,using chafing dish（OR=4.233） and having salted food frequently（OR=8.978）,the poor capability for self-adjustment（OR=7.623）,using chemical fertilizer frequently（OR=2.342）,the history of colorectal disease（OR=5.521） and collateral relatives suffered from colorectal cancers or not（OR=10.729）,while drinking tea related with colorectal cancer negatively（OR=0.201,P〈0.01）.Conclusion Colorectal cancer is significantly associated with having fat and using chafing dish and having salted food frequently,the poor capability of self-adjustment,using chemical fertilizer frequently,the history of colorectal disease and having collateral relatives suffered from colorectal cancers.And drinking tea is a potential protective factor for colorectal cancer.
