摘　　要：目的探讨三叉神经周围支撕脱术治疗三叉神经痛的近期疗效。方法对23例确诊为原发性三叉神经痛者,施行神经撕脱术。第一支1例,第二支11例,第三支11例。结果本组23例,22例有效,1例无效。8例在术后10d有一过性跳疼。术后半年至8年复发的6例中,第二支2例,第三支4例,其中3例又再次手术。结论三叉神经痛周围支撕脱术治疗三叉神经痛近期疗效好,远期疗效还待进一步观察。Objective To explore the short term efficacy of peripheral branch avulsion for treatment of patients with trigeminal neuralgia and clinical features after the operation.Methods Avulsion of supraorbital nerve branch was performed in 23 cases diagnosed as primary trigeminal neuralgia. The nerve decompression tear off box was adopted with eyebrow bow within the frame auricle incision, its margin was near to the hole, then finding out the frame nerve. Although nerve and nerve branch were tore off under intraoperative box from the mouth incision. Two peripheral branches were inside the bone, so wearing a hole in operation might be appropriate for expanding bony hole. Nerve tore off procedure was performed in 23 patients with primary trigeminal neuralgia. Among them, avulsion of first branch was performed in 1 case, avulsion of second branch in 11 cases and avulsion of third branch in remaining 11 cases.Results Among 23 cases, pain had been relieved in 22 cases, but temporary pain recurred in 8 cases at 10 days after the operation. Six cases were relapsed at half a year after the operation, 3 of them were re-operated and 1 case was failed in treatment.Conclusion Peripheral branch avulsion of trigeminal nerve is effective in treatment of trigeminal neuralgia, its short term effectiveness is good, but its long-term effectiveness yet needs further observation.
