上颌异位尖牙的微螺钉种植支抗矫治

摘　　要：目的探讨微螺钉种植体支抗在上颌异位尖牙矫治中的临床应用,以及微螺钉种植体即刻负载所承受的牵引力大小。方法对5例已萌出的上颌异位尖牙直接植入微螺钉种植体,而对4例高位埋伏阻生的异位尖牙则在微螺钉种植体植入前1周或同时行开窗术,显露埋伏阻生的异位尖牙唇侧冠尖1/3,并粘结方丝弓托槽,微螺钉植入后1周左右,在垂直曲式辅弓的支持下进行矫治,在异位的尖牙与微螺钉支抗之间用链状橡皮环加力,使异位尖牙向上、向后(前)移动。开始矫治力在30～50 g之间,此后根据情况逐渐加力,以70 g左右为最佳,最大不宜超过100 g。结果 9例上颌异位尖牙通过微螺钉种植体支抗在垂直式辅弓的帮助下,10～12个月进入正常牙弓,完成矫治。9颗微螺钉种植体无一例中途松动脱落而使矫治失败。结论上颌异位尖牙通过微螺钉种植支抗矫治技术是完全可以排入正常牙列而不必拔除的,保持了患者良好的容貌。Objective To explore the treatment method of the micro-screw implant anchorage in canine ectopic eruption and the traction size of the micro-screw implant with immediate orthodontic loading. Methods The micro-screw implant for five patients with upper canine eetopic eruption was implanted. While for another four patients with canine ectopic impaction of high site, win- dowing to expose its labial top 1/3 crown and bonded the edgewise bracket before the micro-screw implant implanted were applied one week or at the same time. One week latter, with the help of the upright arch, the chain rubber ring between the canine eetopie and the anchorage to move the canine were put. The corrective force was kept between 30 -50 g at first, then increased gradually according to the situation. The optimum force was 70 g,the maximum was below 100 g. Results 9 cases of ectopic canine with the implant and auxiliary arch support had been aligned into the dental arch in 10 - 12 months. None of the 9 micro-screw implants were loose or come off during the treatment period. Conclusion The ectopic canine can be aligned completely into the dental arch by means of the orthodontic technique without the tooth extraction keeping the patients good looks.
