炎症性肠病患者生存质量及其影响因素的调查

摘　　要：目的研究炎症性肠病(IBD)患者的生存质量(QOL)影响因素。方法应用中文IBDQ、SF-36、焦虑自评量表、抑郁自评量表、简易应对方式问卷、社会支持评定量表,对广州市4所医院的71例炎症性肠病患者的QOL进行横断面调查,通过相关分析和多元回归分析,筛选出影响IBD患者QOL的主要因素。结果 IBD患者的生存质量得分明显下降,显著低于正常对照组的生存质量,P<0.05。多元回归分析提示,对生存质量影响的因素为:抑郁、病情、焦虑、病期、年龄。结论 IBD患者的生存质量水平较低,其中年轻(<35岁)、病情重的IBD患者的生存质量较低;缓解期IBD患者主要在心理健康方面的QOL受到影响。焦虑、抑郁水平越高,患者的生存质量越低。Objective To investigate the quality of life（QOL） in patients with inflammatory bowel diseases（IBD） and the factors related to the QOL of patients with IBD. Methods A crass-sectional survey was carried out in four hospitals in Guangzhou city, and 71 IBD patients were recruited in the study. IBDQ, SF-36, Self-rating Anxiety Scale （SAS）, Self-rating Depression Scale （SDS）, Simplified Coping Style Scale and Social Support Questionnaire were used in this study. Demographic data and disease-related indicators were collected. Those data of the IBD patients were compared with that of 103 normal persons. Correlation and multiple regression analysis were applied in data analysis to identify the factors which were related to the QOL of IBD patients. Results The score of QOL in IBD patients was lower than that in normal control group. The factors that impacted on the QOL of the IBD patients were seriated by multiple regression model as depression, severe IBD cases, education level, anxiety, disease relapse and age. Conclusion The QOL of IBD patients was low. The QOL of young patients aged 〈 35 with IBD was lower than that of the older patients. The QOL of severe cases was lower than those mild cases. The QOL of patients in relapse was impaired in all domains ,while the QOL of patients in remission was mainly impaired in psychological and social domains. The QOL of IBD patients with higher anxiety and/or depression level was relatively low.
