总前列腺特异抗原与游离前列腺特异抗原及其比值在前列腺癌鉴别诊断中的价值

摘　　要：目的 探讨血液中总前列腺特异抗原（t-PSA）、游离PSA（f-PSA）以及f-PSA/t-PSA比值对前列腺癌的诊断价值.方法 采用微粒子酶联免疫法（MEIA）检测50例良性前列腺增生症（BPH）和50例前列腺癌患者血清中t-PSA、f-PSA,并计算出f-PSA/t-PSA比值.结果 血清总PSA在4.0～10.0μg/L时,良性前列腺组与前列腺癌组的t-PSA差异无统计学意义[良性前列腺增生组f-PSA为（1.59±0.69）μg/L、f-PSA/tPSA为（0.29±0.06）;前列腺癌组分别为（1.01±0.32）μg/L,（0.17±0.04）,P＜0.05或P＜0.01].结论 当t-PSA在4.0～10.0μg/L诊断灰区时,f-PSA与f-PSA/t-PSA比值对诊断前列腺癌有重要的临床意义.Objective To evaluate the free prostate specific antigenc（f-PSA）/total PSA（t-PSA） value in diagnosis of prostate tumor by integrally testing the serum t-PSA, f-PSA and the ratio of f-PSA/t-PSA. Methods The Serum t-PSA. f-PSA of 50 benign prostutic hyperplasia（BPH）, and 50 prostate cancer（Pca） were measured by particle enzyme immunoassay （EMIA）. Results The level of t-PSA showed no significant difference among the cases of PCa and BPH when it was in the range of 4.0-10.0 μg/L（ P 〉 0.05 ）. A significant difference of f-PSA and f/t ratio was detected among cases of PCa and BPH （P 〈0.05 or P〈0.01）. f/t ≤0.16 were used as the standard for Pca diagnosis, showing a sensitivity of 87.54% and a specificity of 79.24 %. Conclusion When t-PSA level is within 4.0-10.0 μg/L in the gray zone, F/T ratio and f-PSA have a significant value for diagnosing PCa.
